超声评价左心室收缩功能与冠心病实证者辨证分型关系初探

目的:应用超声技术评价左心收缩功能,探索与冠心病心实证者的辨证分型关系,以指导临床治疗。方法:选择冠心病患者101例作为研究对象,其中心血瘀阻证40例,寒凝心脉21例,痰浊闭塞23例,气滞心胸17例,正常对照组30例,对5组研究对象进行超声检测并作对比分析。结果:与正常对照组比较,病变各组IVSae、LVPWae、EF、FS均明显降低,冠心病中医不同证型间心血瘀阻证EF、FS明显低于痰浊闭塞证、寒凝心脉、气滞心胸三证,有显著的统计学意义;痰浊闭塞、寒凝心脉、气滞心胸三证之间IVSae、LVPWae、EF、FS无明显差异。结论:冠心病心实证者辨证分型与左心室收缩功能损害程度有一定关系。

Ultrasonograghic study on left ventricle(LV) systolic function changes of coronary heart disease (CHD) patients in different types of excess syndrome in traditional chinese medicine(TCM)

Objective:To assess LV systolic function of CHD patients with chocardiography.And to study their interrelations in different excess syndrome types.Methods:Totally 101 CHD patients were classified in to five groups,which are differentiated in excess syndrome of TCM,including 40 patients with blood stasi,21 patients with cold coagulation,23 patients with phlegm,17 patients with blockage of Qi,30 for normal control group.LV systolic function of five groups was assessed with echocardiography and was comparatively analyzed.Results:Compared to normal controls,CHD patients with excess syndrome showed significantly decreased LV ejection fraction(EF),LV fractional shortening(FS),inter ventricular septum amplitude of excursion(IVSae)and Left ventricular posterior wall amplitude of excursion(LVPWae);Among different excess syndrome types,patients with blood stasi showed significantly decreased EF,FS.The other four types have no significantly different EF,FS,IVSae and LVPWae.Conclusion:There is a correlation between the excess syndrome types of TCM and LV systolic function.
