乙型肝炎病毒携带者T细胞亚群检测及其与肝组织病变程度的关系

目的观察乙型肝炎病毒(HBV)携带者T细胞亚群的变化及其与肝组织病变程度的关系。方法HBV携带者79例进行肝组织病理检查,应用流式细胞仪检测T细胞亚群,与HBsAg阴性的健康者22例比较分析。结果(1)非活动性HBV携带者与对照组比较,CD4+百分率和CD4+/CD8+比值下降,CD8+百分率升高,差异有统计学意义(P<0.05)。慢性HBV携带者与对照组比较,CD4+、CD4+/CD8+明显下降,CD8+明显升高,差异有统计学意义(P<0.01)。(2)非活动性HBV携带者与慢性HBV携带者CD4+、CD8+百分率与CD4+/CD8+比值比较,差异无统计学意义(P>0.05)。(3)慢性HBV携带者CD4+百分率在G3～4亚组明显高于G0～2亚组(P<0.05),非活动性HBV携带者2亚组间比较,差异无统计学意义(P>0.05)。结论HBV携带者细胞免疫功能紊乱,导致慢性HBV携带。

The relationship between the T lymphocyte subsets expression and pathological changes in liver tissues in HBV carriers

Objective To observe the changes of the peripheral blood T lymphocyte subsets and its relationship with pathological changes in HBV carriers. Methods Histopathological examinations were performed in 79 asympt＇omatic HBV carriers. Flow cytometer was used to detect the peripheral blood T lymphocyte subsets in asymptomatic HBV carriers and 22 controls. Results （ 1 ） The percentage of CD4^＋ cells and the value of CD4^＋/CD8^＋ were significantly reduced in non-active HBV carriers and in chronic HBV carriers than in the normal controls （ P 〈 0.05 or P 〈0.01 ）. However,the percentage of CDs cells was significantly increased （ P 〈0.05 or P 〈0.01 ）. （2） No significant differences of the percentage of CD4^＋ ,CDs cells were observed between nonactive HBsAg carriers and chronic HBV carriers （ P 〉 0.05 ）. （3）Compared with G0-2 ,the percentage of CD4^＋ ceils was significantly increased in G3 -4 in chronic HBV carriers （ P 〈 0. 05 ）. Conclution Patients with asymptomatic HBV carriers are abnormal in cellular immunity, which may be associated with HBV persistent infection.
