肝细胞肝癌患者血清AFP—IgM复合物表达水平及其临床意义

目的探讨肝细胞肝癌（HCC）患者血清甲胎蛋白-IgM免疫复合物（AFP—IgM）的表达水平及其临床意义。方法应用酶联免疫吸附分析（ELISA）法检测79例HCC患者、20例中度肝硬化患者和30例健康对照者血清中AFP-IgM水平，并进行对比分析。结果AFP-IgM的浓度和阳性率分别为HCC组422．20×10^3AU／L和73％，肝硬化组268．85×10^3AU／L和20％，健康对照组119．71×10^3AU／L1和7％。HCC组阳性率显著高于肝硬化组和健康对照组（分别P〈0．01）；Ⅰ期和Ⅱ期HCCAFP-IgM阳性率（分别为89％和88％）高于Ⅲ期（30％，P〈0．05）；AFP-IgM的水平与肿瘤分期、大小有关。结论血清AFP-IgM对HCC的早期诊断具有重要意义，有望成为一个有用的辅助诊断HCC的生物标志。

The expression and significance of serum AFP-IgM immune complexes in hepatocellular carcinoma

Objective To investigate the expression level of AFP-IgM in hepatocellular carcinoma (HCC).Methods The serum AFP-IgM concentrations were detected by ELISA in 79 HCC patients,20 patients affected by liver cirrhosis and 30 healthy controls.Results AFP-IgM concentrations in serum and the positive rate were significantly higher in HCC than that in liver cirrhosis and in healthy controls(P＜0.01).The positive rates of AFP-IgM in the stage Ⅰ or Ⅱ(89%,88%)were higher than that of the stage Ⅲ(30%,P＜0.05).The serum concentration of AFP-IgM is related to the stage,tumor sizes.Conclusion AFP-IgM was of significance for early diagnosis of HCC,it will hopefully be a useful biological marker for the diagnosis of HCC.
