腹腔镜与开腹手术治疗输卵管妊娠后再孕率比较

目的探讨使用不同术式和手术方法治疗输卵管妊娠后患者的再孕率。方法回顾性分析235例输卵管妊娠妇女经手术治疗后的生育状况，比较开腹手术与腹腔镜手术、输卵管保守性手术与切除手术之间的区别。结果宫内妊娠率在开腹组39.2％，在腹腔镜组59.6％，两者差异有显著性（P〈0.01）；重复异位妊娠率在开腹组13.5％，在腹腔镜组10.6％，两者差异无显著性（P〉0.05）。宫内妊娠率在输卵管保守性手术组61.8％，在输卵管切除组29.0％，两者差异有显著性（P〈0.01）；重复异位妊娠率在输卵管保守性手术组11.0％，在输卵管切除组12.9％，两者差异无显著性（P〉0.05）。结论输卵管妊娠患者术后宫内妊娠率，腹腔镜手术明显优于开腹手术，保留输卵管的手术明显优于输卵管切除术，重复异位妊娠率与手术方式及手术类型无关。

Comparative study on pregnancy rates after laparoscopy and laparotomy surgery for tubal pregnancy

Objective To investigate the pregnancy rates after different surgicalmanagement of tubal pregnancy. Methods A retrospective study was carried out to analyse the reproductive out - come of women after different surgical management of tube pregnancy. Results The intrauterine pregnancy rate is 39.2% in laparotomy group and 59.6% in laparoscopy group, There is a statistic significance between laparotomy and laparoscopy group （P〈0.01）. The recurrent ectopie pregnancy rate is 13.5% in laparotomy group and 10.6% in laparoscopy group, there is no correlation between laparotomy and laparoscopy group （P〉0.05）. The intrauterine pregnancy rate is 61.8% in conservative tubal surgery group and 29.0% in salpingectomy group, There is a statistic significance between conservative tubal surgery group and salpingectomy group（ P 〈 0.01）. The recurrent ectopic pregnancy rate is 11.0% in conservative tubal surgery group and 12.9% in salpingectomy group, There is no correlation between conservative tubal surgery group and salpingectomy group （P〉0.05）. Conclusion The intrauterine pregnancy rate after surgery tbr tubal pregnancy, laparoscopy is better than laparotomy in the management of tubal pregnancy. Conservative tubal surgery is better than salpingectomy in the management of tubal pregnancy.
