双介入治疗肝门部胆管癌并发症分析

目的 探讨双介入治疗肝门部胆管癌（HC）的并发症原因及防治措施.方法 回顾性研究肝门部胆管癌患者74例,所有患者均接受经皮肝胆管引流（PTCD）及金属内支架植入术,术后7d在CT引导下行^125 I放射性粒子植入术,观察术后并发症.结果 术后随访6～36个月,出现胆汁性腹膜炎8例,腹泻5例,气胸4例,粒子迁移3例,肝出血1例,胆汁瘤2例.结论 双介入治疗肝门部胆管癌疗效显著,术后并发症是影响手术效果的主要因素,针对病因积极预防和及时处理并发症对提高手术效果、改善患者生活质量极其重要.

Analysis of Complication of Double Interventional Therapy in the Bile Duct Carcinoma of Porta Hepatic

Objective To discuss the reason and the prevention and control measures in the treatment of Complication of Double Interventional Therapy in the bile duct carcinoma of porta hepatic. Methods Retrospective study 74 cases patients with bile duct carcinoma of porta hepatic, all patients were received PTCD and metal stent implantation operation,postoperative seven days leaded by CT and received ^125I Radioactive particles implantation and observe the postoperative complications. Results All patients were followed up for 6 to 36 months. Eight cases had biliary peritonitis, diarrhoea in five cases, pneumothorax in 4 cases, particle immigration in three cases, stent slippage in two cases,hepatic bleeding in one cases, bile tumor in two cases. Conclusion PTCD and metallic biliary stents placement combined with ^125I radioactive particles implanatation can get better therapeutic efficacy,the postoperative eomplieations was the main factors that influenced surgical effect. To prevente the pathogeny and cure of the complications in time is very importance to improve operation effect and improve the patient of life quality.
