摘    要:目的观察椎一基底动脉供血不足(VBI)性眩晕患者前庭刺激对后组颅循环的影响,探讨自主神经对后组颅循环血流动力学的神经调节功能。方法选取VBI性眩晕患者50例(眩晕组)和正常志愿者50例(对照组),采用左耳灌入30℃水50ml作为前庭刺激方法,应用头颅多普勒超声记录刺激前后左椎动脉(LVA)、右椎动脉(RVA)、基底动脉(BA)的收缩峰速度(Vs)、舒张末峰速度(Vd)、平均血流速度(Vm)。结果前庭刺激前眩晕组与对照组比较,BA各项参数有所降低,但无统计学意义(P〉0.05),LVA、RVA的Vs、vd、Vm均明显低于对照组(P均〈0.05);前庭刺激后眩晕组RVA、BA的Vs、Vd、Vm均低于对照组(P均〈0.05),而LVA的Vd低于对照组(P〈0.05);两组前庭刺激前、后各项参数增高的百分率比较,仅BA的Vs、Vm变化比对照组明显降低(P〈0.05)。结论VBI性眩晕患者后组颅循环中的BA神经调节功能差,使其临床易发生VBI。

Abstract:Objective To study the influence posterior cerebral circulation in vertebro-basilar insufficiency (VBI) vertigo patients by vestibular stimulation, and to explore autonomic nervous regulation of posterior cerebral circulation. Methods The experiment took vertebro-basilar insufficiency patients (the vertigo group) and healthy people (the control group) as our study objects, the vertigo group consisted of 50 patients, the control group consisted of 50 persons. Vestibular stimulation was made by pouring 50 ml and 30℃ water into left ear. The basilar artery(BA) and bilaterally the vertebral artery (VA) hemodynamics indices were examined before and after stimulation. Such as systolic peak flow velocity (Vs) , end-diastolic peak flow velocity (Vd), mean peak flow velocity(Vm). Results The indices in the BA decreased in the vertigo group than that of the control group before vestibular stimulation, these changes were no statistical significant difference( P 〉 0.05). Vs, Vd, Vm in the LVA, RVA were lower in the vertigo group than the control group(P 〈 0.05 ) , and Vs, Vd, Vm in the RVA, BA and Vd in the LVA were lower than that of the control group after, vestibular irrigation ( P 〈 0.05 ). Vs, Vd, Vm of RVA, BA in vertigo group were lower than that of the control group after vestibular irrigation ( all P 〈 0.05 ), but Vd of LVA was lower than that of the control group ( P 〈 0.05 ). Percentage changes of the parameters were compared after vestibular irrigation, only Vs, Vm in the BA decreased significantly in the vertigo group than that of the control group ( P 〈 0.05 ). Conclusions Nervous regulation function in the BA of posterior cerebral circulation is bad in vertigo patients with VBI, and it easily makes VBI happen in the clinical.
