甲腈吡酮、黄夹次甙乙及地高辛强心作用的比较

摘    要:戊巴比妥钠引起的心力衰竭犬,静注甲腈吡酮(milrinone,Mil)0.05,0.1mg/kg及地高辛(digoxin,Dig)0.05mg/kg后,左室压力上升、下降速率(±LVdp/dt max)和左室收缩力(LVCF)明显增加。这表明有明显正性肌力和正性舒张作用。静注Mil 0.1mg/kg后,动脉平均压和左室舒张末期压均降低,心率稍加快;0.05mg/kg对上述指标无影响。静注Dig 0.05mg/kg后血压升高;十二指肠注入0.2mg/kg的Mil及黄夹次甙乙(neriifolin,Ner)亦有明显的强心作用。同剂量的Dig则作用不明显。普萘洛尔(心得安)引起的心衰豚鼠在静脉恒速注射Mil、Ner及Dig后均有强心作用。Ner的治疗量最小,Mil的安全范围最宽。Dig和Ner均能引起心律失常,Mil的此种作用不明显。

Abstract:Milrinone is a potent cardiac bipyridine with inotropic and vasodilating properties. In pentobarbital-induced heart failure in anesthetized dog, intravenous injection of milrinone (0.05, 0.1mg/kg) or digoxin(0.05mg/kg) increased the maximum rate of pressure change ( LVdp/dt max)and cardiac contractile force (CF). After intravenous injection of milrinone (0.1 mg/kg) MAP end LVEDP were decreased, however, no effect was observed at a dosage of 0.05 mg/kg. On the contrary, arterial blood pressure was increased when digoxin was injected at a dosage of 0.05mg/kg. Intraduodenal administration of milrinone or neriifolin (0.2 mg/kg) showed significant cardiotonic effect; but no obvious effect when digoxin was given at the same dosage. In propranolol-induced failing heart of anesthetized guinea pig, intravenous infusion of milrinone, neriifolin and digoxin exhibited potent cardiotonie effect. The therapeutlc dosage of neriifolin was found to be lower than that of milrinone and digoxin. The safety margin of milrinone is much greater than that of neriifolin and digoxin.
