甲状腺眼病眼外肌厚度及其与眼部血流动力学相关性的研究

摘    要:目的:探讨彩色多普勒超声在甲状腺相关性眼病眼外肌厚度测量中的应用价值及甲状腺相关眼病患者眼外肌厚度与视网膜中央动脉血流参数的相关性。方法:采用彩色多普勒超声诊断仪测量并记录45例甲状腺相关眼病患者(90只眼)的眼外肌厚度并计算其平均值、受累情况,并进行视网膜中央动脉血流参数的测量,分析正常对照组与甲状腺眼病组眼外肌厚度与视网膜中央动脉血流参数的相关性。结果:甲状腺相关眼病组眼外肌厚度明显增厚;CDFI显示视网膜中央动脉收缩期峰值流速增加,舒张期血流速度降低,测值与对照组有显著变化(P〈0.01)。结论:甲状腺相关眼病患者眼外肌厚度与视网膜中央动脉血流参数之间具有正相关性;彩色多普勒超声测量甲状腺相关眼病患者眼外肌厚度安全准确。

Abstract:Objective:To evaluate the value of color doppler ultrasound in examining the thickness of extraocular muscles and the relationship between the extraocular muscles and hemodynamics in thyroid ophthalmopathy.Methods:The extraocular muscles and blood flow of the central retinal artery(CRA)were examined by color doppler ultrasound for 45 Graves' disease patients with exophthalmus(GDEgroup).The relationship between the extraocular muscles and hemodynamics were compared between the GDE and the normal control(NC)groups.Results:Compared with the NC group,The thickness of extraocular muscles were thicker,the peak systolic velocity(PVS)was increased,the peak diastolic velocity(PVD)was decreased in the CAR and the retrobulbar data were changed significantly(P<0.01)。Conclusion:CRA blood flow had positive correlation to thickness of extraocular muscles;Color doppler ultrasound examination is a safe,accurate method of diagnosing thyroid ophthalmopathy.
