指动脉转位与传统静脉移植在复杂断指再植中的应用效果对比

目的 分析复杂断指再植中指动脉转位与传统静脉移植的修复效果。方法 68例复杂断指再植患者,根据手术方案不同分为对照组与实验组,各34例。对照组接受传统静脉移植治疗,实验组接受指动脉转位移植治疗。对比两组患者手指功能评分、一次移植成活率、指腹两点辨别距离及治疗满意度、断指修复效果。结果 实验组美观度评分(14.25±2.98)分、运动功能评分(34.68±3.67)分、血液循环状态评分(7.28±1.24)分、感觉恢复评分(15.09±3.06)分及一次移植成活率94.12%均高于对照组的(11.35±3.41)分、(31.07±3.05)分、(5.25±1.04)分、(12.34±2.54)分、76.47%,指腹两点辨别距离(4.23±0.85)mm明显小于对照组的(6.11±0.62)mm,差异均有统计学意义(P<0.05)。实验组治疗总满意度94.12%高于对照组的73.53%,差异有统计学意义(P<0.05)。实验组断指修复优良率97.06%明显高于对照组的79.41%,差异有统计学意义(P<0.05)。结论 复杂断指再植中应用指动脉转位效果显著,其一次性移植成活率、治疗满意度均相对较高,且手指功能恢复较好。

Comparison of digital artery transposition and traditional vein transplantation in replantation of complex severed fingers

Objective To analyze the repair effect of digital artery transposition and traditional vein transplantation in replantation of complex severed fingers.Methods A total of 68 patients with replantation of complex severed fingers were divided into control group and experimental group by different surgical methods,with 34 cases in each group.The control group was treated by traditional vein transplantation,and the experimental group was treated by digital artery transposition.The finger function score,survival rate of onetime transplantation,identification distance between two points of finger abdomen and satisfaction degree of treatment,repair effect of severed finger was compared between the two groups.Results The beauty score(14.25±2.98)points,motor function score(34.68±3.67)points,circulatory status score(7.28±1.24)points,sensory recovery score(15.09±3.06)points,survival rate of one-time transplantation 94.12% in the experimental group was higher than(11.35±3.41)points,(31.07±3.05)points,(5.25±1.04)points,(12.34±2.54)points and 76.47% in the control group,and identification distance between two points of finger abdomen(4.23±0.85)mm was obviously smaller than(6.11±0.62)mm in the control group.Their difference was statistically significant(P<0.05).The total satisfaction of treatment 94.12% in the experimental group was higher than 73.53% in the control group,and the difference was statistically significant(P<0.05).The excellent-good rate of repair of severed fingers 97.06% in the experimental group was obviously higher than 79.41% in the control group,and the difference was statistically significant(P<0.05).Conclusion Digital artery transposition shows remarkable effect in replantation of complex severed fingers.The survival rate of one-time transplantation and the satisfaction degree of treatment are relatively high,and the function of fingers recovers well.
