经阴道四维超声造影在子宫输卵管通畅性评估中的临床价值

目的 探究经阴道四维超声造影(TVS-4D-HyCoSy)在子宫输卵管通畅性评估中的价值。方法 90例不孕症患者分别予以TVS-4D-HyCoSy检查、经阴道二维超声造影检查(TVS-2D-HyCoSy),以腹腔镜检查结果为金标准,比较两种检查方法的诊断效果、输卵管通畅率及不良反应发生情况。结果 TVS-4D-HyCoSy检查的特异性为92.86%(26/28)、敏感性为95.65%(22/23)、诊断符合率为94.12%(48/51),均高于TVS-2D-Hy Co Sy检查的71.43%(20/28)、73.91%(17/23)、72.55%(37/51),差异均有统计学意义(P<0.05)。TVS-2D-Hy Co Sy检查输卵管通畅率为68.89%(62/90), TVS-4D-HyCoSy检查输卵管通畅率为66.67%(60/90),比较差异无统计学意义(P<0.05)。TVS-2D-Hy Co Sy和TVS-4D-Hy Co Sy检查的不良反应发生率(4.44%VS 6.67%)比较差异无统计学意义(P>0.05)。结论 不孕症患者予以TVS-4D-Hy Co Sy检查,能够准确评估输卵管通畅性,效果良好,且不良反应少,有助于保证患者检查安全,值得作为优选检查技术。

Clinical value of transvaginal four-dimensional hysterosalpingo-contrast sonography in the assessment of fallopian tube patency

Objective To investigate the value of transvaginal four-dimensional hysterosalpingo-contrast sonography(TVS-4D-HyCoSy)in the assessment of fallopian tube patency.Methods A total of 27 infertile patients were examined with TVS-4D-HyCoSy and two-dimensional hysterosalpingo-contrast sonography(TVS-2D-HyCoSy)respectively.Based on the results of laparoscopy as the gold standard,the diagnostic effect,patency rate of fallopian tube and the occurrence of adverse reactions were compared between the two methods.Results The sensitivity,specificity and diagnostic coincidence rate by TVS-4D-HyCoSy examination were 92.86%(26/28),95.65%(22/23)and 94.12%(48/51)respectively,which was higher than 71.43%(20/28),73.91%(17/23)and 72.55%(37/51)by TVS-2D-HyCoSy examination,and their difference was statistically significant(P<0.05).The patency rate of fallopian tube was 68.89%(62/90)by TVS-4D-HyCoSy examination,which was 66.67%(60/90)by TVS-4D-HyCoSy examination,and the difference was not statistically significant(P<0.05).There was no statistically significant difference in incidence of adverse reactions by TVS-2D-HyCoSy and TVS-4D-HyCoSy examination(4.44%VS 6.67%)(P>0.05).Conclusion TVS-4D-HyCoSy examination in patients with infertility can accurately evaluate the patency of fallopian tube with good effect and few adverse reactions,which is helpful to ensure the safety of patients’examination.It can be used as the optimal examination technique.
