直肠黏膜套扎治疗直肠黏膜内脱垂所致的出口梗阻型便秘的效果观察

目的 探究直肠黏膜套扎治疗直肠黏膜内脱垂所致的出口梗阻型便秘的效果。方法 72例直肠黏膜内脱垂所致的出口梗阻型便秘患者,按照接诊顺序奇偶性分为观察组及对照组,各36例。对照组患者采用直肠黏膜下硬化剂注射术治疗,观察组患者采用直肠黏膜套扎术治疗。对比两组患者的临床疗效及治疗前后焦虑症状积分、便秘症状积分、直肠肛门测压指标水平。结果 观察组患者的临床治疗总有效率为94.44%(34/36),显著高于对照组的69.44%(25/36),差异具有统计学意义(P<0.05)。治疗后,两组患者的纳差、焦虑、痞满、失眠等焦虑症状积分均低于治疗前,且观察组低于对照组,差异均具有统计学意义(P<0.05)。治疗后,两组患者的排便时间、排便费力、排便不尽感以及大便性状等便秘症状积分均低于治疗前,且观察组低于对照组,差异均具有统计学意义(P<0.05)。治疗后,对照组患者的肛管静息压、肛管舒张压、直肠静息压、肛管最大收缩压水平均低于本组治疗前和观察组治疗后,差异均具有统计学意义(P<0.05);观察组治疗前后的肛管静息压、肛管舒张压、直肠静息压、肛管最大收缩压水平比较差异均无统计学意义(P>0.05)。结论 针对直肠黏膜内脱垂所致的出口梗阻型便秘患者,采用直肠黏膜套扎治疗,可显著改善患者的生活质量,取得极佳的治疗效果,可改善患者负性情绪,加快患者康复速度,具有临床应用价值。

Observation on the effect of rectal mucosal ligation in the treatment of outlet obstruction constipation caused by internal rectal mucosa prolapse

Objective To investigate the effect of rectal mucosal ligation in the treatment of outlet obstruction constipation caused by internal rectal mucosa prolapse.Methods A total of 72 cases of outlet obstruction constipation caused by internal rectal mucosa prolapse were divided into observation group and control group by admission order,with 36 cases in each group.The control group was treated with rectal submucosal sclerosant injection,and the observation group was treated with rectal mucosal ligation.The clinical efficacy,anxiety symptoms score,constipation symptoms score and anorectal manometry before and after treatment was compared between the two groups.Results The total effective rate of clinical treatment was 94.44%(34/36)in the observation group,which was significantly higher than 69.44%(25/36)in the control group,and the difference was statistically significant(P<0.05).After treatment,the scores of anxiety symptoms such as poor appetite,anxiety,fullness and insomnia in the two groups was lower than those before treatment,and the observation group was lower than the control group.Their difference was statistically significant(P<0.05).After treatment,the scores of constipation symptoms such as defecation time,laborious defecation,feeling of endless defecation and stool property in the two groups was lower than those before treatment,and the the observation group was lower than the control group.Their difference was statistically significant(P<0.05).After treatment,the anal resting pressure,anal diastolic pressure,rectal resting pressure,anal maximum systolic pressure in the control group was lower than those before treatment and those in the observation group after treatment.Their difference was statistically significant difference(P<0.05).There was statistically significant difference in anal resting pressure,anal diastolic pressure,rectal resting pressure,anal maximum systolic pressure before and after treatment in the observation group(P>0.05).Conclusion For patients with outlet obstruction constipation caused by internal rectal mucosa prolapse,rectal mucosal ligation shows excellent treatment effect,and it can significantly improve the quality of life of patients,improve their negative emotions,and speed up their rehabilitation.It has clinical application value.
