RAUMDIC双腔脑室外引流管治疗蛛网膜下腔出血的应用研究

目的：探讨RAUMDIC双腔脑室外引流管治疗蛛网膜下腔出血的临床应用研究。方法：入选临床蛛网膜下腔出血患者396例,分为对照组198例,治疗组199例。对照组予以常规药物及单腔脑室外引流管治疗;治疗组给予常规药物及RAUMDIC双腔脑室外引流管治疗,分别观测比较2组患者治疗后1、3、5、7d的脑脊液红细胞计数。统计比较术后14d内患者手术并发症的发生率和颅内感染率;术后7、14d的体温及GCS评分变化。结果：治疗组患者术后脑脊液检验红细胞数较对照组降低明显（P〈0.05）,术后感染率小于对照组（P〈0.05）,手术并发症发生较对照组无差异（P〉0.05）,治疗后患者体温恢复快（P〈0.05）,治疗后14d的GCS昏迷评分较对照组高（P〈0.05）。结论：RAUMDIC双腔脑室引流管较传统单腔脑室引流管提供了更为准确真实的颅内压监测,减少了颅内感染,提高了SAH患者的临床治疗效果。

Clinical Study of RAUMDIC Double-lumen Ventricular Drainage Catheter for Subarachnoid Hemorrhage(SAH)

Objective To study the effect of double-lumen ventricular drainage catheter for subarachnoid hemorrhage（SAH）.Methods 396 patients with SAH were divided into two groups randomly,including the 199-cases treatment group with RAUMDIC double-lumen ventricular drainage catheter and the 198-cases control group with single-lumen ventricular drainage catheter-control group.Both the two groups accepted conventional medicine treatment.The control group used single-lumen catheter for ventricular drainage,while the treatment group used double-lumen catheter.The change of RBC count in CSF was tested at 1d,3d,5d and 7d after ventricular drainage surgery.After 2 weeks,the intracranial infection cases and puncture-associated complications cases were concerned,and the changes of Glosgow Coma Scores（GCS） and the patients＇ body temperature were evaluated.Results Compared with that in the control group,the RBC count in CSF decreased conspicuously in the double-lumen catheter group after ventricular drainage （P0.05）.The double-lumen catheter group had a lower intracranial infection proportion and better GCS and the patients＇ body temperature recovery than the single-lumen catheter group 14 days later（P 0.05）.The puncture-associated complications were not increased in the double-lumen catheter group（P0.05）.Conclusion Compared with the single catheter,RAUMDIC double-lumen ventricular drainage catheter can reduce the proportion of intracranial infection after ventricular drainage surgery,and has better benefit for patient＇s ICP monitor and recovery.
