临床路径在腹腔镜胆囊切除术住院患者中的应用与评价

目的探讨实施临床路径对新型腹腔镜胆囊切除术患者临床效益的影响。方法纳入因胆囊结石合并慢性胆囊炎住院行腹腔镜切除术患者60例，均分为临床路径组30例（优化组）和非临床路径组30例（普通组），临床路径组采用改进临床路径法，非临床路径组实施常规法，观察比较两组患者平均住院日、平均住院费用、住院手术并发症等指标，统计两组间有无差异。结果纳入临床路径患者住院费用比未纳入者下降2255．23元，平均住院日缩短1．83天，术前等待时间缩短2．22天。结论新型临床路径具有良好的实用性，能够在保证医疗安全的情况下提高各项医疗指标，提高医疗效益，解决病人看病贵的问题，值得进一步推广应用。

New clinical pathway applied to improve healthcare quality in surgery

Objective To evaluate the effect of Laparoscopic cholecystectomy of a new clinical pathway on healthcare quality. Methods Thirty patients underwent original clinical pathway for in-patient in West China Hospital. Thirty patients underwent new clinical pathway for in-patitent in West China Hospital. I compared the hospitalization cost, inhospital days, and stay time before operation of in-patient between these two groups. Results Compared with in-hospital days of the two group, the new clinical pathway group with original clinical pathway group decreased 1.83days（P〈 0.05）. The stay time before operation decreased 2.22 days（P〈0.05）. The hospitalization cost decreased 2255.23 RMB （P〉0. 05）. Conclusion The new clinical pathway is practicality which can improve the bed turnover rate effectively, decrease patients; stay time before operation and decreased of hospitalization cost.
