煤矿重型颅脑损伤并发多器官功能不全综合征临床分析

摘    要:目的 对煤矿重型颅脑损伤并发多器官功能不全综合征(MODS)进行分析,为临床早期诊断、治疗和预防MODS提供帮助.方法 对1995年7月-2007年12月我院收治的32例重型颅脑损伤并发MODS患者的临床资料进行回顾性分析,总结其临床特点和治疗方法.结果 重型颅脑损伤并发MODS的发生率为7.82%,肺、胃肠是最常见的受累器官,其次为脑、心、肾、肝脏,病死率为59.37%,且病死率与受累器官的个数密切相关.结论 格拉斯哥昏迷(GCS)评分低、高龄、低氧血症是重型颅脑损伤并发MODS的重要原因.煤矿重型颅脑损伤并发MODS病死率高,预后差.加强院前急救、避免诱因和早期预防是降低MODS发生的重要因素.

Severe Craniocerebral Injury Complicated with Multiple Organ Dysfunction Syndrome in Coal Miners
Abstract:Objectives To summarize clinical features of 32 patients with coal mine severe craniocerebral trauma (CCT) complicated with multiple organ dysfunction syndrome(MODS) to provide aids in early diagnoses,treatments and prevention of multiorgan system failure (MOSF).Methods A retrospective study was performed on coal mine severe CCT complicated with MOSF in 32 patients.Results The incidence rate of MODS was 7.82%.The commonest organs involved were lung and gastrointestinal tract,followed by cardiovascular,kidney and liver,with a mortality of 59.37%,and the mortality was closely related to the number of involved organs.Conclusions Low Glasgow Coma Scale (GCS) score,old age,hyoxemia are the important reasons for severe CCT complicated with MODS.The mortality of coal mine CCT with MODS is high,with a poor prognosis.Strengthening pre-hospital first-aid,avoiding precipitating factors and early prevention can reduce the occurrence of MOSF.
