2型糖尿病伴牙周病患者经牙周病基础治疗后血清瘦素和糖化血红蛋白水平的变化

目的探讨2型糖尿病伴牙周病患者，经牙周病基础治疗后，其血清瘦素和糖化血红蛋白水平的变化。方法选取50例糖尿病伴牙周炎患者，随机分为两组，试验组实施牙周病基础治疗，对照组暂不进行任何牙周病治疗，并于治疗前和治疗后1个月、3个月分别测量并记录牙周袋探诊深度（probing depth，PD）、附着水平（attach．mentlevel，AL）、血清瘦素和糖化血红蛋白的值，对两组数据进行比较分析。结果试验组PD、AL、血清瘦素治疗前分别为（6．73±0．82）mm、（3．25±L07）mm、（1．0，2±0，84）ng／mL，治疗后1个月分别为（4．88±0．91）mm、（1．96±1．01）mm、（0．86±0．39）ng／mL，治疗后3个月分别为（4．54±0.78）mm、（1．83±0.99）mm、（0．67±0．28）ng／mL。试验组PD、AL、血清瘦素水平治疗后1个月和3个月较治疗前均有显著降低，差异有统计学意义（P〈0．05），血清糖化血红蛋白水平治疗后3个月较治疗前下降，差异有统计学意义（P〈0．05）.对照组患者PD、AL、血清瘦素和血清糖化血红蛋白水平研究期间变化无统计学差异（P〉0．05）。结论牙周病基础治疗可降低糖尿病伴牙周炎患者的血清瘦素和糖化血红蛋白水平。

Effect of basic periodontal therapy on the level of serum leptin and HbAlc in type 2 diabetes patients with periodontitis

Objective The aim of this study was to evaluate the efficiency of basic periodontal therapy on the level of serum leptin and HbA1c in type 2 diabetes patients with periodontitis. Methods Fifty patients with type 2 diabetes and periodontitis, were randomized into two groups, and basic periodontal therapy were given to patients in test group, while patients in control group were not given any therapy. At baseline, 1 and 3 months after treatment, the PD, AL, serum leptin and HbAlc were studied, and then compare the difference between one and three months after treatment and baseline. Results In test group, the PD, AL and serum leptin in the baseline were （ 6. 73 ± 0. 82） mm, （ 3.25 ±1.07 ） ram, （ 1.02 ± 0.84） ng/mL, respectively ; one month after treatment, the PD was （4.88 ±0. 91 ） mm. the AL was （ 1.96 ± 1.01 ） mm, the serum leptin was （0. 86 ± 0. 39 ） ng/mL; three months after treatment, the PD was （4.54 ± 0.78 ） mm,, the AL was （1.83 ±0. 99 ） mm, the serum leptin was （0.67 ± 0.28 ） ng/mL. In test group, the PD, AL and serum leptin one month after treatment and three months after treatment were significantly lower than that in the baseline （ P 〈 0.05 ） ; and the serum HbA1c of three months after treatment was significantly lower than that in the baseline （ P 〈 0.05 ）. In control group, there was no significant difference between the baseline, one or three months af- ter treatment in PD, AL, leptin and HbAlc. Conclusion The basic periodontal therapy can decrease the serum leptin and HbAlc in type 2 diabetes patients with periodontitis.
