3D-FIESTA-C联合3D-TOF-MRA序列在面肌痉挛微血管压迫的诊断价值

摘    要:目的评价3D-FIESTA-C联合3D-TOF-MRA序列在诊断原发性面肌痉挛微血管压迫的应用价值。方法回顾性分析经微血管减压术(MVD)的84例原发性面肌痉挛患者的影像学资料,所有患者均采用3D-FIESTA-C和3D-TOF-MRA序列扫描,以MVD手术结果为金标准,对比分析3D-FIESTA-C、3D-TOF-MRA序列及序列组合对面肌痉挛责任血管的显示能力。结果与MVD术中结果对照分析显示,3D-FIESTA-C检出责任血管70例,其阳性率83.3%;3D-TOF-MRA序列检出责任血管68例,其阳性率81.0%。3D-FIESTA-C联合3D-TOF-MRA序列共检出责任血管79例,其阳性率94.0%;面肌痉挛责任血管的种类以小脑前下动脉(48.8%,41/84)和小脑后下动脉(26.2%,22/84)为主。结论3D-FIESTA-C联合3D-TOF-MRA序列应用能有效提高对原发性面肌痉挛责任血管的显示。

Abstract:Objective To evaluate the value of showing the responsible blood vessels for neurovascular compression in patients with hamificial spasm by 3D-FIESTA-C and 3D-TOF-MRA sequences.Methods The imaging data of 84 patients with hamificial spasm were analyzed retrospectively.After MRI examination of 3D-FIESTA-C with 3D-TOF-MRA,all of the patients underwent micro-vascular decompression(MVD).Taking surgical results as the gold stands,comparative analysis was performd to display the abitity of the responsible blood vessels by 3D-FIESTA-C and 3D-TOF-MRA sequences in patients with hamificial spasm.Results Compared with surgical results,3D-FIESTA-C showed presence of the responsible blood vessels in 70 cases with a positive rate of 83.3%.3D-TOF-MRA showed presence of the responsible blood vessels in 68 cases with a positive rate of 81.0%.And 79 cases were found when combining both 3D-FIESTA-C and 3D-TOF-MRA sequences,with a positive rate of 94.0%.The main offending vessels were anterior inferior cerebellar artery(48.8%,41/84)and posterior inferior basilar artery(26.2%,22/84).Conclusion The 3D-FIESTA-C combined with 3D-TOF-MRA sequences application can increase the display of responsible blood vessels in patients with hamificial spasm.
