二维、三维超声心动图在房间隔缺损封堵术中的对比性研究

摘    要:目的探讨经胸二维超声心动图(TTE)、经食道超声心动图(TEE)及实时三维超声心动图(RT-3DE)在房间隔缺损(ASD)封堵术中的应用及其对比性分析。方法选取烟台毓璜顶医院2018年5月~2019年11月62例ASD,比较患者术前TTE、TEE与RT-3DE测量的ASD最大直径及面积(三维测量)与术中封堵器直径、面积。结果TTE、TEE、RT-3DE测量房间隔缺损最大径与封堵器直径线性关系分别为:y=5.07+1.08×x,r=0.873(P<0.001);y=2.84+1.08×x,r=0.915(P<0.001);y=0.09+1.08×x,r=0.957(P<0.001)。RT-3DE测量面积与封堵器面积线性关系:y=0.79+1.06×x,r=0.934(P<0.001)。结论二维超声心动图对大多数的ASD选择合适的封堵器型号是可靠的,并且使用更加简单方便。RT-3DE对于复杂的ASD能够更快的作出评估。RT-3DE测量缺损处面积对于选择封堵器型号也是可行的。

Abstract:Objective To explore the application and comparative study of two-dimensional transthoracic echocardiography(TTE),transesophageal echocardiography(TEE)and real-time three-dimensional echocardiography(RT-3DE)in transcatheter closure of atrial septal defect(ASD).Methods 62 patients with ASD in Yu Huang Ding Hospital of Yantai from May 2018 to November 2019 were selected.The maximum diameter of ASD and the diameter of occluder measured by TTE,TEE and RT-3DE were compared.The area measured by RT-3DE and the area of occluder were compared.Results The linear relationship between the diameter measured by TTE,TEE and RT-3DE and the diameter of occluder was respectively y=5.07+1.08×x,r=0.873(P<0.001);y=2.84+1.08×x,r=0.915(P<0.001);y=0.09+1.08×x,r=0.957(P<0.001).Linear relationship between area measured by RT-3DE and occluder was y=0.79+1.06×x,r=0.934(P<0.001).Conclusion Two-dimensional echocardiography is reliable for most ASD to choose the appropriate occluder model,and it is more simple and convenient to use.RT-3DE can evaluate complex ASD more quickly.The measurement of defect area by RT-3DE is also feasible for selecting the type of occluder.
