中央型小细胞肺癌与肺鳞癌CT征象分析

目的分析中央型小细胞肺癌(small cell lung cancer,SCLC)及肺鳞癌(squamous carcinoma,SC)的CT表现,探讨各自影像学特点及鉴别诊断价值。方法选取2019年1月~2019年12月在我院经病理证实的SCLC及SC患者的临床及影像学资料,分为SCLC组30例,SC组31例,对两组肺癌患者的CT征象进行统计学分析。结果纵隔及肺门淋巴结肿大、淋巴结融合、支气管狭窄无阻塞、血管受侵、“沼泽样”强化在SCLC中出现概率高于SC组(P值均<0.05);支气管闭塞、阻塞征象及“湖泊样”强化在SC中出现概率高于SCLC组(P值均<0.05);其他征象均差异无统计学意义(P值均>0.05)。结论中央型SCLC与肺SC的CT征象差异在两者的诊断及鉴别诊断中具有一定价值。

Analysis of CT signs of central small cell lung cancer and lung squamous carcinoma

Objective To analyze the CT imaging findings of central small cell lung cancer and lung squamous carcinoma,and to explore their imaging characteristics and differential diagnostic value.Methods The clinical and imaging data of patients with central small cell lung cancer and squamous carcinoma confirmed by pathology in our hospital from January 2019 to December 2019 were collected.They were divided into SCLC group of 30 cases,SC group of 31 cases.Statistical analysis of CT signs of lung cancer patients was conducted.Results Mediastinal hilar lymphadenopathy,lymph node fusion,bronchial stenosis without obstruction,vascular invasion,"marsh-like"enhancement were more likely to occur in SCLC(all P<0.05);The probability of occurrence of bronchial occlusion,obstruction signs,and"amber-like"enhancement was higher in SC than SCLC(all P<0.05).There were no statistical differences in other signs(all P>0.05).Conclusion The difference in CT signs between central SCLC and SC has certain value in the diagnosis and differential diagnosis of the two.
