双水平无创正压通气治疗COPD合并呼吸衰竭的疗效

目的研究BiPAP呼吸机在慢性阻塞性肺疾病(COPD)合并呼吸衰竭中的疗效。方法将25例COPD合并呼衰患者随机分为治疗组13例,应用BiPAP呼吸机治疗;对照组12例,持续氧疗及应用呼吸兴奋剂处理,两组同时抗感染等常规治疗,比较两组动脉血气结果、呼吸频率、心率、插管率、病死率及住院时间等观察指标。结果治疗组2～3h及24h血气分析中的PH值、PCO2、PO2均较入院时改善且差异有显著性(P<0.05),而对照组与入院比较差异无显著性;其中理论插管率治疗组(2/13)较对照组(7/12)低且差异有显著性(P<0.05);理论住院时间治疗组较对照组短且差异有显著性(P<0.05)。结论双水平无创正压通气可迅速改善COPD合并呼吸衰竭病人的血气情况,减少插管率与住院时间,是治疗呼衰的有效手段。

The effect of BiPAP in treating COPD complicated with respiratory failure

Objective To research the effect of BiPAP in treating COPD complicated with respiratory failure. Methods Thirteen cases were randomly chosen as the study group treated by BiPAP breathing apparatus, and the other 12 cases as controls treated by lasting oxygen therapy and pneumatic analeptic. All other treatments were equal for both groups. Results The levels of blood gas analysis in the study group including PH, PaCO2 and PaO2 significantly improved compared with pre-therapy (P<0.05), while in the control group it was negtive. Compared with the controls, the theory frequency of cannulas and the theory length of staying in the study group were better than those in the control group (P<0.05).Conclusion BiPAP can quickly improve the blood gas conditions of COPD complicated with respiratory failure, reduce the frequency of cannulas and length of stay in the hospital.It is a good method to cure respiratory failure.
