住院老年痴呆患者监护人的心理状态及其影响因素

目的探讨住院老年痴呆患者监护人的心理状态及其影响因素。方法采用一般情况调查表、症状自评量表（SCL-90）、社会支持评定量表（SSRS）、简易智力状态检查（MMSE）及AD病理行为评分表（BEHAVE-AD）对住院老年痴呆患者及门诊老年痴呆（AD和VD）患者和其监护人进行评定，并与正常老人的监护人和老年非痴呆患者的护理人员进行比较。结果住院痴呆患者监护人SCL-90总分及因子分比正常对照组和老年非痴呆患者护理组评分高，但较门诊痴呆组评分低，均有显著差异（P〈0．01-0．05）。单因素分析发现不同性别、年龄、文化程度、婚姻状况、工作情况、性格、病程、与患者的关系及有无躯体疾病之间SCL-90总评分均具有非常显著差异（P〈0．01）：逐步回归分析显示影响监护人心理状态的因素依次为患者痴呆的严重程度、精神行为症状、与患者关系、性别及社会支持。结论住院老年痴呆患者监护人的心理状态较差，影响因素是多方面的，应引起社会高度重视。

Study of mental health status of the guardian of the senile dementia in hospital

Objective To explore the mental health status and influencing factors of the guardian of the senile dementia in hospital.Method The scales SCL-90,SSRS,MMSE and BEHAVE-AD were used in the senile dementia in hospital or out-patient and the guardian,and contrast the guardian of healthy controls,the guardian of nursing patient. Results It showed that SCL-90 scores were much higher in the guardian of the senile dementia in hospital than those guardians of healthy controls and nursing patient.But were lower than those guardian of out-patient(P<0.01~0.05);The single factor analysis discovered that the different sex,age,cultural degree,marital status,work circumstance,personality,course of illness,with the sufferer's relation and have a grade point with total SCL-90 of have no body disease to all have to show the difference of very much(P<0.01);Analysis of multiple stepwise regressions showed the main factors influenced guardian mental health status were MMSE,BEHAVE,with patient relation,the sex,SSRS.Conclusion The mental health status of the guardian of the senile dementia in hospital are all lower.The affecting factors of the mental health status is vavious,which need to cause the high value.
