MR扩散加权成像(DWI)在胆总管下段病变中的应用

摘    要:目的探讨MR扩散加权成像(DWI)及表观扩散系数(ADC)值在胆总管下段病变诊断的价值。方法对50例正常志愿者及100例胆总管下段病变患者行常规MR及DWI检查,并测量ADC值。包括胆总管结石45例,十二指肠乳头炎症21例,恶性占位性病变34例。结果正常十二指肠乳头ADC值为(1.88±0.41)×10^-3mm^2/s;胆总管结石、十二指肠乳头炎、恶性占位性病变ADC值分别为(1.92±0.38)×10^-3mm^2/s、(2.14±0.08)×10^-3mm^2/s、(1.61±0.29)×10^-3mm^2/s,胆总管下段恶性占位性病变与胆总管结石、十二指肠乳头炎ADC值存在统计学意义(P〈0.05)。结论分析DWI图像特点及ADC值,其在胆总管下段病变的诊断及鉴别诊断中有重要价值。

Abstract:Objective To evaluate the efficacy of MR diffusion-weighted imaging (DWI) and apparent diffusion coefficient (ADC) values in the diagnosis of the common bile duct lesions. Methods Conventional MRI and DWI were performed in 50 healthy subjects and 100 cases with common bile duct lesions, including 45 cases of choledocholithiasis, 21 cases of duodenal inflammation, 34 cases of malignant lesions. The ADC values were measured. Resuits The mean ADC value of normal duodenal was(1.88 ± 0.41) × 10^-3mm^2/s. The mean ADC value of choledocholithiasis,duodenal inflammation and malignant lesion were(1.92 ± 0.38) × 10^-3mm^2/s, (2.14 ± 0.08)× 10^-3mm^2/s,(1.61 ± 0.29)× 10^-3mm^2/s respectively. The differences of the mean ADC values between choledocholithiasis and duodenal inflammation, malignant lesion were significant(P〈0.05). Corldusion MR DWI and ADC values were useful in diagnosis of the common bile duct lesions.
