肺癌刷落细胞、活检组织及痰液细胞端粒酶的活性研究

摘    要:目的探讨支气管镜下肺刷落细胞、活检细胞及痰液细胞的端粒酶活性检测对诊断肺癌的价值。方法采用TPAP结合银染色定性法 ,对 46例肺部疾病患者支气管镜下肺刷落细胞、活检组织及其新鲜痰液细胞的端粒酶活性进行分析。结果32例肺癌患者 ,其活检组织、肺刷落细胞及痰液细胞端粒酶阳性率分别为 90 .6 %、87.5 %、5 3 .1% ,显著高于肺炎性、良性增生组 ;与病理学检查结果相比 ,肺刷落细胞及痰液细胞的端粒酶阳性率明显高于其细胞涂片中的癌细胞检出阳性率。结论痰液细胞及支气管镜下肺刷落细胞、活检组织的端粒酶活性检测 ,可作为肺癌诊断的有效而灵敏的指标 ,能提高肺癌的检出率。

Abstract:ObjectiveTo evaluate the detection of telomerase activity insputum cells, bronchoscopic brush-off cells and bioptic tissues for diagnosis of lung cancer. Methods Telomerase activity in fresh sputum cells, bronchoscopic brush-off cells and bioptic tissues from 46 patients with pul monary diseases were examined by TRAP-silver assay. Results Positive rates of telomerase activity in sputum cells, bronchoscopic brush-off cells and bioptic tissues from 32 lung cancer patients were 53.1%, 87.5% and 90.6% respectively, significantly higher than those in patients with pneumonic and benign proliferat ion (P<0.01).In comparison with the results of pathological examination, the positive rates of tolomerase activity in the sputum cells and exfoliated cells obtained from fiberbronchoscopic brushing in the 32 lung cancer patients were distinctly higher than those of cancer cell detection on cytological smears(P <0.01). Conclusions Estimation of telomerase activity in sputum cells, bro nchoscopic brush-off cells and bioptic tissues from lung may bean effective and sensitive biomarker for diagnosis of primary lung cancer, increasing its dete ction rate.
