胃癌侵犯胃裸区:一个新的预后因素

目的　探讨胃癌侵犯胃裸区 (gastricbarearea ,GBA)的CT表现和解剖病理基础 ,分析其与预后的可能关系。资料与方法　回顾性分析连续 4 0例胃癌患者的CT资料 ,按解剖部位分为近侧胃癌 (proximalgastriccancer,PGC)组和远侧胃癌 (distalgastriccancer,DGC)组。观察胃癌侵犯GBA的CT表现特征 ,比较PGC组和DGC组胃癌侵犯GBA出现率的差异。结果　 9例胃癌侵犯GBA病例中 ,有 8例CT表现为GBA内出现软组织肿块或结节状淋巴结。PGC组中 ,胃癌侵犯GBA的出现率为 4 3.8% ,而在DGC组仅为 8.3% ,两者有明显差异 (P <0 .0 5 )。结论　PGC侵犯GBA的出现率高于DGC组。胃癌侵犯GBA有一定的CT表现特征。GBA受累和腹膜后淋巴结转移可能与PGC的预后较差有关

Bare Area Involvement by Gastric Carcinoma:A Notable Prognostic Factor

Objective To study the CT features of bare area involvement by gastric carcinomas and their anatomic pathological basis, and to analyze the relation between CT manifestations and prognosis.Materials and Methods 40 consecutive gastric carcinoma cases, which divided into proximal gastric carcinoma (PGC) group and distal gastric carcinoma (DGC) group according to anatomic position of primary tumor, were retrospectively studied. CT images and incidence of gastric bare area (GBA) involvement,and compared the differeance between DGC group and PGC group were observed.Results In 9 cases with bare area involvement, irregular soft tissue density masses or lymph nodes in the GBA were demonstrated in 8 cases. The incidence of GBA involvement in PGC group was 43.8%, significantly difference from those in DGC group (8.3%) (P<0.05).Conclusion The incidence of GBA involvement in PGC group was significantly higher than those in DGC group (P<0.05). There are some CT image features in the GBA involvement. Bare area involvement by gastric carcinoma and lymphatic spread in retroperitoneal space might have relationship with poor prognosis.
