中医疗法治疗2型糖尿病伴发抑郁疗效观察

目的:探讨中医疗法对2型糖尿病伴发抑郁患者的疗效,为中医疗法临床应用提供依据。方法:将入组的2型糖尿病患者114例分为中医治疗组、心理治疗组、对照组,采用空腹血糖(FBG)、空腹胰岛素(FINS)、糖化血红蛋白(HbA1C)、胰岛素抵抗指数(ISI)评估疗效,症状自评量表(SCL-90)、汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)评定心理状态,采用中医证候评分判定疗效。结果:治疗前后比较:中医治疗组FBG、FINS、HbA1C、ISI4种生化指标的比较差异均有统计学意义(P<0.05),中医治疗组及心理治疗组SCL-90总分、躯体化、抑郁、焦虑因子分、HAMD总分、HAMA总分的比较差异均有统计学意义(P<0.05);组间比较:治疗后,中医治疗组FBG、FINS、HbA1C、ISI4种生化指标显著低于心理治疗组及对照组(P<0.05),中医治疗组及心理治疗组SCL-90总分、躯体化、抑郁、焦虑因子分、HAMD总分、HAMA总分显著低于对照组(P<0.05);中医治疗组、心理治疗组及对照组中医证候疗效差异有统计学意义(P<0.05)。结论:中医疗法可以有效治疗糖尿病伴发抑郁症状,改善情绪状态,降低血糖和胰岛素抵抗,值得临床医师注意。

Clinical Observation on TCM Therapy Treating Type 2 Diabetes Complicated with Depression

Objective：To study the efficacy of Chinese medicine therapy on patients with type 2 diabetes complicating depression and to provide the basis of clinical applications of Chinese medicine therapy.Methods：114 patients with type 2 diabetes complicating depression were divided into Chinese medicine treatment group,psychotherapy group and the control group.The treatment efficacy was assessed by the fasting blood glucose（FBG）,fasting insulin（FINS）,glycated hemoglobin（HbA1C）,insulin resistance index（ISI） and Chinese medicine syndromes.The psychological state was assessed by symptom checklist（SCL-90）,Hamilton Depression scale（HAMD） and Hamilton Anxiety Scale（HAMA）.Results：There were significant differences of the Chinese medicine treatment group before and after treatment in FBG,FINS,HbA1C and ISI（P〈0.05）.There were significant differences of the Chinese medicine treatment group and psychotherapy group before and after treatment in SCL-90 total score,somatization,depression,anxiety factor score,HAMD and HAMA total score（P〈0.05）.There were significant differences among the Chinese medicine treatment group,psychotherapy group and the control group in FBG,FINS,HbA1C,ISI,SCL-90 total score,somatization,depression,anxiety factor score,HAMD,HAMA total score and Chinese medicine syndromes after treatment（P〈0.05）.Conclusion：Chinese medicine therapy can be effective on treating patients with type 2 diabetes complicating depression and improving emotional state,lowering blood sugar and insulin resistance.
