针灸治疗颈椎病133例疗效观察及分析

目的:观察针灸治疗颈椎病的疗效,分析疾病分型、证型的疗效和痊愈疗程。方法:从疾病分型和证型上对133例颈椎病临床疗效及痊愈病例疗程评价,比较两型的证型构成。结果:总病例的治愈率为45.86%,好转率为45.11%,总好转率为90.97%。在痊愈天数上,颈型为(12.50±5.34)天,神经根型为(16.59±4.66)天;且风寒湿阻最短,痰湿阻络其次,气血虚弱、气滞血瘀、肝肾不足较长。在疗效上,颈型比神经根型要好,痊愈率分别58.67%,29.31%;风寒湿阻型最好,痰湿阻络其次,肝肾不足型最差。证型构成上,颈型与神经根型存在显著差异。结论:针灸治疗颈椎病有较好的疗效,疾病分型、证型及证型构成对疗效、疗程及愈后判断有一定的意义。

Observation and Analysis of Therapeutic Effect on 133 Cervical Spondylosis Cases Treated by Acupuncture

Objective：To observe therapeutic effects of acupuncture on cervical spondylosis,and analyze both therapeutic effect in disease subtypes and TCM syndrome and treatment course.Methods：The therapeutic effect of acupuncture on 133 cases of cervical spondylosis was analyzed from not only disease subtypes but also TCM syndrome,and time of therapy in two subtypes was estimated in the cured cases.Results：The cured rate,improvement rate and total improvement rate were 45.86%,45.11% and 90.97% in all cases.In time of therapy,the neck subtype and radiculopathy subtype were（16.59±4.66）days,（12.50±5.34）days respectively,still,cold and dampness syndrome was the shortest,phlegm-dampness blocking meridians syndrome is the next,and the syndromes of deficiency of both Qi and blood,Qi stagnantion and blood stasis,deficiency of both liver and kidney have the longest treatment course.In therapeutic effect,the neck subtype cured rate was 58.67% which was better than the radiculopathy subtype（29.31%）,moreover,cold and dampness syndrome was the best,phlegm-dampness blocking meridians syndrome was the next,and deficiency of both liver and kidney syndrome was the worst.In the composition of syndrome patterns,there was significant difference between the two subtypes（P≤0.05）.Conclusion：For treating cervical spondylosis,acupuncture is very effective,and disease subtypes,TCM syndrome and the composition of syndrome pattern are certainly significant in judging therapeutic effect,course of treatment and prognosis.
