冠状动脉造影阳性患者平板运动试验结果分析

目的分析冠状动脉(冠脉)造影(CAG)阳性患者CAG与平板运动试验(TET)结果,评价无创性TET在冠心病诊断中的价值。方法回顾分析2002年1月至2008年11月住院行CAG确诊冠心病并于CAG前两周内行TET检查的313例患者,对比CAG与TET结果,分析假阴性原因。结果 (1)313例CAG阳性患者中,TET阳性171例(54.6%),可疑阳性97例(31.0%),阴性45例(14.4%),(2)TET阳性患者的冠脉病变较重,且多为多支病变。(3)假阴性率为14.4%,这部分患者的冠脉病变较轻且多为单支病变。结论 TET用于评价心肌缺血及诊断冠心病时具有较高的敏感度,且简便易行,仍是目前诊断冠心病常用检查方法之一。但TET存在一定的假阴性率,评价TET结果应结合患者病史、年龄、冠心病易患因素、运动量及伴随症状进行综合分析,以提高TET诊断冠心病的准确性。

Analysis of treadmill exercise stress test in patients with coronary heart disease confirmed by coronary angiography

Objective To compare the results of treadmill exercise test （TET） and coronary angiography（CAG） in patients with coronary heart disease （CHD）. Methods The results of TET and CAG were retrospectively reviewed in 313 patients with CHD. TET was performed within 2 weeks before CAG for each patient. Results Of 313 patients with CHD, 171 patients （54. 6% ） had positive, 97 patients （31.0%） had suspectively positive and 45 patients （ 14. 4% ） had negative TET results. Most of the patients with positive TET results had multiple vessel disease and severe lesions of coronary artery. The false-negative rate was 14, 4% and most of the patients with false-negative had 1-vessel disease. Conclusions TET is a simple, practical and non-invasive method in the diagnosis of CHD.
