54例喉咽癌临床分析

目的 探讨喉咽癌外科治疗的手术方法选择。方法 对我院１９８８～１９９６年间收治５４例喉咽癌的外科治疗效果进行回顾性分析。结果 ５年生态２率３７．０％（２０／５４），其中游离空肠移植３７。．５％，胃咽吻合的３７．５％，胸大肌肌皮瓣为５０％，全喉切除或局部皮瓣修昨为３５％，无手术死亡病例。并发症发生率为３５．２％（１９／５４），全部病人恢复吞咽功能，结论 对晚期喉咽癌选择游主空肠移植或胃咽吻合为好。

Clinical analysis of 54 cases of hypopharyngeal cancer

Objective To investigate the surgical treatments for hypopharyngeal cancer.Methods 54 cases of hypopharyngeal cancer were analyzed;all the patients had received surgical treatment from 1986 to 1996.Results The 5 year survival rate was 37%(20/54)in total.The 5 year survival rate for those reconstructed with a free jejunal graft was 37.5%,for those with pharyngogastric anatomosis 37.5%,for those with a pectoralis major myocutaneous flap 30%,and for those subjected to total laryngectomy and reconstructed with a local flap 35%.These patients all survived the operation and regained their swallowing function.The rate of complication was 35.2%(19/54).Conclusion Reconstruction with a free jejunal graft or with the pharyngogastric anatomosis could be used with more effective results and there fore might be the choice of surgical treatments for the advanced cases of hypopharyngeal cancer.
