摘 要: | 目的:为了改善传统开放颌下腺切除术后的美观效果,探讨内窥镜辅助颌下小切口颌下腺切除术的有效性、安全性和可行性。方法:回顾性分析30例内窥镜辅助经颌下小切口行颌下腺切除术患者的临床资料,术前诊断慢性颌下腺炎7例(4例含涎石),多形性腺瘤16例,颌下腺囊肿7例,所有病例术前经影像学、三维超声学评估。结果:所有患者均于内镜辅助下完全切除颌下腺和肿瘤,无一例中转开放手术,无手术并发症,术后美观效果好,患者满意度较高,多形性腺瘤患者术后随访时间4~36个月,未见复发。结论:内镜辅助经颌下小切口入路行颌下腺切除术安全可行,与传统开放手术相比,美容效果好,颌部切口瘢痕小,无面神经下颌缘支损伤风险。

Abstract: | Objective To improve the beautiful effect of the traditional open resection of the submandibular gland,and to probe the effectiveness,safety and feasibility of endoscope-assisted resection of submandibular gland incision.Method The clinical date of 30 cases of patients who had submandibular gland excision through endoscope-assisted small incision was analyzed retrospectively.Preoperative diagnosis of chronic submandibular gland inflammation had 7 cases(4 cases with salivary stone),pleomorphic adenoma had 16 cases,submandibular gland cyst had 7 cases.All cases were assessed by preoperative imaging,three-dimensional ultrasound study.Results All patients removed submandibular gland and tumor through endoscopic-assisted resection completely,with no conversion to open surgery,no surgical complications,aesthetic effect,and all patients were satisfied.Pleomorphic adenoma patients were followed up for 4 to 36 months,no recurrence.Conclusion Endoscopic-assisted small incision through the line of submandibular submandibular gland resection is safe and feasible,compared with traditional open surgery,it has cosmetic results,jaw,small scar,no marginal mandibular branch of facial nerve injury risk. |
