体外反搏对缺血性心脏病左室重塑的影响

目的:探讨体外反搏改善缺血性心脏病左室重构的作用和机理。方法:对50例经冠脉造影证实为缺血性心脏病患者,常规治疗基础上随机分为体外反搏组和常规治疗组(对照组),在治疗前、治疗后半年和一年分别测量心胸比率和左心室各项参数,对所得数据进行比较分析。结果:与治疗前对比,反搏组半年后心胸比率和左心室各项参数有显著下降(P<0.05),一年后部分参数差异更加显著(P<0.01);而对照组则在一年后才有显著差异(P<0.05);反搏组与对照组比较,心胸比率和左室各项参数均有显著改善(P<0.05),部分参数一年后差异更加显著(P<0.01)。结论:体外反搏较一般常规治疗能更早更明显地改善缺血性心脏病左室重构。

Effect of EECP on the Left Ventricular Remodling in Ischemic Heart Disease

Objective: To evaluate the effects and mechanism of enhanced external counterpusation(EECP) on the left ventricular remodling in ischemic heart disease. Method: 50 patients(male 38, female 12) with ischemic heart disease were divided into EECP and routine treatment groups radomly. Sternum and echocardiography were performed before and 6/12 month after the EECP. Results: The rate of heart/chest and the parameter of left ventricle ameliorated 6/12 month later dropped in EECP group(P<0.05/P<0.01) .especially the 12 month later. The rate of heart/chest and the parameter of left ventricle ameliorated dropped until 12 month later in routine group(P<0.05). Conclusion: Compared with routine treatment, EECP can ameliorate the left ventricular remodling earlier and more obviously.
