脊髓海绵状血管瘤MRI与病理对照

目的 :探讨脊髓海绵状血管瘤的 MRI特征。方法 :经 MRI诊断、活检及手术病理证实脊髓海绵状血管瘤 6例 ,其中活检 3例 ,手术 3例 ,分析了该组病变的分布范围 ,MRI异常信号特征 ,并与病理切片进行对照研究。结果 :6例脊髓海绵状血管瘤中 4例位于胸段 ,2例位于颈段 ,均为单发 ,脊髓粗细正常或稍增粗。 MRI特征性表现为瘤巢呈爆米花状或桑葚状混合信号团块 ,周围可见短 T2 低信号环。镜下瘤组织由高度扩张的血窦所构成 ,内衬扁平内皮细胞 ,壁薄 ,管腔内可见红细胞 ,血窦间为少许纤维组织分隔 ,并见新旧出血病灶。结论 :MRI诊断脊髓海绵状血管瘤特异性高 ,对病灶不同时期变化如出血、含铁血黄素沉积、胶质增生均具有特征性表现

The Control Study of Magnetic Resonance Image and Pathology for Spinal Cavernous Hemangioma

Objective:To investigate the characteristic of magnetic resonance image(MRI)for spinal cavrnous hemangioma.Methods:6 cases of spinal cavernous hemangioma diagnosed by MRI and confirmed by biopsy or operation pathology were reviewed.The distribution region of lesion and MRI abnormal signal feature was analysed and compared with the pathological section.Results:There were 4 cases lesions in thoracic segment of the spinal cord and 2 in cervical segment.All of the lesion was solitary.The shape of the spinal cord was normal or slight thick.The MRI characteristic of the hemangioma was just like popcorn or mulberry mixed signal lump and short T 2 signal could be found around the lesion.The hemangioma was made up of superectatic blood sinusiod under the microscope and the wall was thin and lined with flat epithelial cell.There were little fibrous tissue and hemorrhagic lesions mixed in the blood sinusoid and some erythocytes in the lumen. Conclusion:MRI was specific in diagnosis of spinal cavernous hemangioma.Pathologic changes of hemangioma in different period such as bellding,deposit of hemosiderin and colloid hyperplasia could be presented on MRI.
