大鼠不稳定膀胱的产生与毒蕈碱受体亚型密度改变

摘    要:背景:不稳定膀胱的发生与毒蕈碱受体(MR)变化有关,MR有5种亚型,而膀胱逼尿肌仅有两种亚型—M2R,M3R,目前尚不清楚MR亚型在不稳定膀胱发生中的作用。目的:探讨MR亚型密度变化与不稳定膀胱发生之间的关系。设计:随机对照的实验研究。单位:解放军第三军医大学西南医院泌尿中心。材料:实验在重庆西南医院中心实验室完成,选用为30只Wistar大鼠。方法:建立不稳定膀胱大鼠模型,将30只大鼠分成实验组和对照组各15只,实验组在10g/L苯巴比妥麻醉下,对大鼠施行尿道结扎手术,造成尿道不完全梗阻。对照组大鼠仅切开皮肤,而不结扎尿道。采用RT-PCR方法测定实验组大鼠不稳定膀胱及对照组逼尿肌MR亚型密度。主要观察指标:尿动力学,M2RNA和M3RNA含量,及两者的比值变化。结果:对照组逼尿肌组织含有M2RNA和M3RNA,M2RNA(1.67±0.42)含量明显多于M3RNA(0.64±0.19),两者之比约2.59∶1,没有检测到M1,M2和M3亚型的mRNA。实验组大鼠不稳定膀胱逼尿肌中M2和M3密度均增加,分别为(2.03±0.65)和(1.53±0.46),M3增加更明显(P<0.01),两者比值显著缩小,约1.4∶1。结论:不稳定膀胱产生与MR亚型上调及M2R,M3R亚型之间的比例失调有关。

Abstract:BACKGROUND: The generation of unstable bladder correlates with the alteration of muscarinic receptor(MR). MR has 5 subtypes in total but only two subtypes exist in cystic detrusor: M2R and M3R. The effect of MR subtype in the generation of unstable bladder is unknown at present.OBJECTIVE: To study the relationship between alteration of MR subtype density and the generation of unstable bladder.DESIGN: A randomized controlled trial.SETTING: Urology Center of Southwest Hospital, Third Military Medical University of Chinese PLA.MATERIALS: The study was conducted in the central laboratory of Southwest Hospital, Chongqing. Thirty Wistar rats were selected.METHODS: The unstable bladder rat model was established in 30 rats,which were divided into control group and study group with 15 rats each. Rats of study group received operation of urethral ligation after anesthesia with 10 g/L of Phenobarbital to produce incomplete urethral occlusion. No urethral ligation was performed in rats of control group, which were only received skin incisions. The density of MR subtype in detrusor of both study group and control group was assayed with RT-PCR technique.MAIN OUTCOME MEASURES: Urinary dynamics, M2RNA and M3RNA contents, and the ratio of them.RESULTS: There were only M2RNA and M3RNA in the detrusor of control group. The M2RNA content(1.67+0.42)]was significantly more than that of M3RNA (0.64 ±0.19)] with the ratio of 2.59: 1. No M1RNA, M4RNA and, M5RNA were detected. The densities of both M2RNA and M3RNA in study group increased to(2.03 + 0.65 ) and (1.53 + 0.46) respectively with more significant increase in M3(P< 0.01), and the ratio of M2 to M3 was significantly reduced to about 1.4: 1.CONCLUSION: The generation of unstable bladder is correlated with the up-regulation of MR subtypes and the imbalance of the ratio between MR subtypes.
