肺硬化性血管瘤8例影像学表现

背景]分析肺硬化性血管瘤的临床特征及影像学表现,以提高诊断水平.病例报告]对8例经手术病理学检查证实的肺硬化性血管瘤的临床和影像学资料进行回顾性分析,8例中女性为7例,男性为1例;年龄为14~63岁,平均为43岁;发生于左肺上叶为1例,左肺下叶为3例,右肺上叶为2例,右肺下叶为2例.CT平扫显示4例肿块边界光整,4例轻度分叶;7例密度均匀,1例密度欠均匀;1例可见钙化及囊变,2例可见空气半月征;增强扫描显示肿块大部分中度、高度强化.经影像学检查发现病灶到手术确诊平均时间为8个月;术前诊断为良性肿瘤为4例,其余4例考虑为恶性肿瘤.讨论]肺硬化性血管瘤是少见的肺部良性肿瘤,临床无特异性表现,CT增强扫描有一定的特征性,而手术切除肿瘤是明确诊断和治疗的有效方法.

Imaging diagnosis of 8 cases of pulmonary sclerosing hemangioma

BACKGROUND To analyze clinical and imaging representation of pulmonary sclerosing hemangioma(PSH) to improve the ability of clinical diagnosis.CASE REPORTS The clinical and imaging data of 8 cases of PSH with histologically confirmed were retrospectively analyzed.Of 8 cases,there were 7 females and 1 male,the age ranged from 14 to 63 years old(mean age,43 years old) at the time of operation.1 case occurred in the superior lobe of left lung,3 cases inferior lobe of left lung,2 cases in the superior lobe of right lung,and 2 cases in the inferior lobe of right lung.On unenhanced CT,the PSH showed well defined and smooth contour in 4 cases,mild lobulation contour in 4 cases,relatively homogeneous in 7 cases and heterogeneous attenuation in 1 case,calcification and cyst in 1 case,the air crescent sign in 2 cases,and on contrast enhanced CT,most part of the tumor showed moderate marked heterogeneous enhancement.Imaging studies showed a lesion to the operation time of diagnosis of 8 months on average.Preoperative impression of benign tumor was made in 4 cases,whereas malignant tumor was suspected in the remaining 4 cases.DISCUSSION PSH is a rare benign tumor of the lung with non-specific clinical features.Surgical removal of the tumor preserving as much lung parenchyma as possible is indicated for proper diagnosis and treatment of this condition.
