BiPAP 呼吸机辅助治疗高龄慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者的护理

目的探讨双水平气道正压（bilevelpositiveairpressure，BIPAP）呼吸机辅助治疗慢性阻塞性肺疾病（COPD）合并Ⅱ型呼吸衰竭患者的护理效果。方法将120例COPD合并型呼吸衰竭患者分成治疗组和对照组，两组均采用常规综合治疗，治疗组在此基础上加用BiPAP呼吸机辅助治疗，观察两组患者治疗前后的动脉血气分析。结果治疗组在治疗7天后PaO2、PaCO2均较治疗前明显改善（P〈0．01），与对照组比较，差异有统计学意义（P〈0．05）。两组均未见呼吸机相关性不良反应。结论应用BIPAP呼吸机能明显提高COPD合并呼吸衰竭的临床疗效，易操作，保持通畅的呼吸道及并发症的预防是护理工作的重点。

Nursing care of elder COPD patients with type 2 respiratory failure treated with BiPAP venti-lator adjunctive therapy

Objective To evaluate the nursing efficacy for chronic obstructive pulmonary disease （COPD） patients with type 2 respiratory failure treated with BiPAP ventilator adjunctive therapy. Methods One hundred and twenty COPD patients with respiratory failure were randomly divided into therapy or control group,60 patients in each group. Patients in the control group were given conven- tional treatment and care,while patients in the therapy group were given BiPAP ventilator adjuvant therapy and care measures on the ba- sis of the control group. The changes of PaO2 and PaCO2 were compared between the groups. Results The levels of PaO2 and PaCO2 of patients in the therapy group were significantly improved after 7 days of treatment, and the improvement was better when compared to the control group （ P 〈 O. 05）. No adverse reaction of ventilator was observed in the either groups. Conclusion BIPAP ventilator can significantly improve the clinical efficacy of COPD patients with respiratory failure, and easy to be operated. It is the focus of nursing work to maintain easy and smooth of the respiratory tract in the course of treatment and prevent the complication.
