儿童股骨颈骨折65例报告及治疗方法评价

对65例儿童股骨颈骨折,采用不同方法进行治疗观察,经4～14年随访,结果44例骨折愈合,股骨头无坏死,髋关节功能正常,占67.7％;19例骨折愈合,股骨头缺血坏死,占29.2％,3例髋内翻,2例髋外翻,2例骨不愈合。认为:新鲜骨折,应采用手法复位鳞纹钉或克氏针固定或牵引加石膏固定,不宜使用三翼钉固定;陈旧性骨折手术效果不佳;骨折内翻位愈合,行外展截骨术效果满意。

Femoral-neck fracture in children.A report of 65 cases and evaluation of different therapies

The clinical observation was carried out on 65 cases of childrens femoral-neck fractures treated with random different methods.The follow up ranging from 4 to 14 years showed that fracture healing, no necrosis of the femoral head and normal function of the hip were obtained in 44 cases (67.7% of the total),fracture healing and ischemic ecrosis of the head in 19(29.3% of the total),hip varus in 3,hip valgus in 2 and no fracture healing in 2.We hold that for fresh fracture fin nailing is unsuitable and should first be selected manual reduction whorled-or Kirschner's-pin fixation,or skeletal traction and plaster fixation;for old fracture no good results were obtained by surgery but abduction osteotomy may lead to satisfactory results for old fracture with varus fracture healing.
