血清铁蛋白联合4种肿瘤标记物对肺癌的诊断价值

目的:探讨血清铁蛋白(SF)联合CA125、CYFRA21-1、癌胚抗原(CEA)、神经元特异度烯醇化酶(NSE)4种肿瘤标记物对肺癌的临床诊断意义。方法:对143例肺癌患者(肺癌组)、56例良性肺部疾病患者(良性肺病组)和40例健康体检者(对照组)的SF、CA125、CYFRA21-1、CEA、NSE水平进行检测,并比较3组各指标的阳性表达率以及5项指标单一或联合检测的敏感度和特异度。结果:肿瘤组患者CA125、CYFRA21-1、CEA、NSE及SF水平均明显高于对照组以及良性肺病组,差异均有统计学意义(P<0.05);对照组、良性肺病组各标记物、SF比较差异均无统计学意义(P>0.05)。肺癌组患者CA125、CYFRA21-1、CEA、NSE及SF阳性率明显高于对照组以及良性肺病组(P均<0.05),不同组织类型肺癌各肿瘤标记物水平差异有统计学意义(P<0.05)。SF联合CA125、CYFRA21-1、CEA、NSE检测可提高肺癌诊断的敏感度(P<0.05)。结论:SF联合其他4种肿瘤标记物检测可提高肺癌的临床诊断率,并对肺癌的病理分型提供依据。

Value of serum ferritin combining with four other tumor markers in the diagnosis of lung cancer

Objective: To investigate the value of serum ferritin(SF) combining with CA125,CYFRA21-1,CEA,NSE in the diagnosis of lung cancer.Methods: SF level and markers of CA125,CYFRA21-1,CEA,NSE in 143 lung cancer cases(lung cancer group) 56 cases with benign lung diseases(benign lung disease group),and 40 healthy volunteers(control group) were detected.Positive rate of the four makers and SF levels were compared among the three groups,and individual and combined sensitivity and specificity of 5 parameters were also compared.Results:Lung cancer group showed higher CA125、CYFRA21-1、CEA、NSE and SF levels as well as higher positive rate of these indicators than the benign lung disease group and the healthy control group(Pboth <0.05),but there’s no significant difference in these levels between the latter two groups(P>0.05).Significant differences in the positive rate of CA125,CYFRA21-1,CEA,NSE and SF were observed in different types of lung cancer.Combined detection of SF and CA125,CYFRA21-1,CEA,NSE levels can raise the specificity(P<0.05).Conclusions: Detection of serum ferritin in combination with other four indicators can improve the diagnostic accuracy,and might be evidence for different pathological type of lung cancer.
