急诊救治糖尿病酮症酸中毒患者30例临床分析

目的观察急诊门诊就诊糖尿病酮症酸中毒患者的临床救治。方法 30例符合糖尿病酮症酸中毒诊断标准的急诊救治的患者,在补液、祛除诱因及纠正电解质紊乱、酸碱平衡等治疗的基础上均给予小剂量胰岛素静脉滴注,观察疗效。结果其中28例患者血糖在24h内降至5.6～11.4mmol/L,血钾、钠、血尿素氮、二氧化碳结合力均恢复正常,尿糖(+)或消失,尿酮体转阴,电解质恢复正常。2例昏迷者神志转清,病情平稳后收入内科病房继续治疗。结论正确应用胰岛素是抢救成功的关键,同时纠正电解质紊乱、防治诱因是治疗成功的重要环节。

The Clinical Analysis about 30 Patients with Diabetic Ketoacidosis by Emergency Treatment

Objective Observation of emergency treatment of patients with the clinical treatment of diabetic ketoacidosis. Methods 30 cases of diagnostic criteria of diabetic ketoacidosis in the emergency treatment of patients,in the rehydration,wipe out incentives and correcting electrolyte imbalance,acid-base balance,on the basis of the treatment were given a small dose of insulin infusion to observe the effect. Results 28 patients within 24h blood sugar dropped 5.6 ～ 11.4mmol/L,serum potassium,sodium,blood urea nitrogen,carbon dioxide binding capacity returned to normal,urine sugar（＋） or disappeared,Ketone body overcast,electrolytes returned to normal. Two cases of coma consciousness turned clear and stable condition after the income of a medical ward for further treatment. Conclusion Insulin is the proper application to save the key to success,while correcting electrolyte disorders,prevention incentives is an important part of successful treatment.
