DISCO CE-MRA在上肢血管疾病中的应用价值研究

Differential sub-sampling with cartesian ordering(DISCO)contrast-enhanced magnetic resonance angiography in preoperative assessment of anterolateral thigh flap

Objective:To evaluate the value of differential sub-sampling with cartesian ordering(DISCO)contrast-enhanced magnetic resonance angiography(CE-MRA)in upper extremity vascular diseases.Methods:MR images of twenty patients with suspected upper limb vascular disease between May 2020 and April 2021 were retrospectively analyzed.All the patients underwent DISCO CE-MRA examination.Among them,11 patients underwent DSA and 2 patients underwent surgery.The DISCO CE-MRA images of all patients were reconstructed by Maximum Intensity Projection(MIP).Combined with conventional MRI sequences,the diagnostic accuracy of DISCO CE-MRA in upper extremity vascular diseases was examined based on DSA and pathological results.Results:In the all 20 patients,there were 10 cases of cavernous hemangioma,2 cases of arteriovenous malformations,1 case of multiple aneurysms with arteriovenous malformations and arteriovenous fistula,1 case of simple arteriovenous fistula,1 case of polydactyly with vascular dysplasia,1 case of proximal right subclavian artery occlusion,1 case of scaphoid fracture postoperatively,and 3 cases of non-vascular disease.Among the 10 cases of cavernous hemangioma,8 cases were confirmed by DSA,1 case was confirmed by surgery and pathology,and 1 case did not undergo further diagnosis and treatment.Additionally,1 case of arteriovenous malformation,1 case of multiple aneurysm with arteriovenous malformation and arteriovenous fistula,and 1 case of simple arteriovenous fistula were confirmed by DSA.1 case of polydactyly with vascular dysplasia was confirmed by surgery.Conclusion:The DISCO CE-MRA technique was an accurate and safe imaging technique.For patients with suspected upper extremity vascular disease,the DISCO CE-MRA examination,combined with MRI routine sequences,might help physicians accurately diagnose and comprehensively evaluate the nature,location and size of the disease.
