Clinical value of SPECT/CT quantitative analysis in the diagnosis of cervical and thoracic bone lesions in tumor patients

Objective To evaluate quantitatively with SPECT SUV,objectively compare the uptake of 99m TC-MDP between spinal cervicothoracic osteoarthritis and bone metastasis,and to explore the diagnostic value of SUV quantification in osteoarthritis and bone metastasis.Methods Fifty-seven patients diagnosed with tumors at our hospital were retrospectively analyzed and treated with 99m TC-MDP for SPECT/CT bone imaging.A total of 187 hypermetabolic cervical and thoracic lesions were detected by bone imaging,including 143 in the bone metastasis group and 44 in the osteoarthritis group.Meanwhile,SUVmax values of normal lumbar vertebrae were taken for each patient as the control group(57).The quantitative analysis of SUVmax in 187 lesions was carried out.Results The SUVmax levels of the control group,osteoarthritis group and bone metastasis group were(9.1±2.0),(13.9±5.6)and(22.1±8.8).The SUVmax level of the bone metastasis group was significantly higher than that of the osteoarthritis group and the control group,and the SUVmax level of the osteoarthritis group was significantly higher than that of the control group,with statistically significant differences(P<0.01).According to ROC curve analysis,the area under the curve(AUC)of the bone metastasis group-control group(AUC=0.922)was the highest,the AUC value of the osteoarthritis group-control group and the bone metastasis group-osteoarthritis group was 0.884 and 0.826,respectively.The ROC curve showed an AUC of 0.769.The SUVmax level of non-osseous bone metastasis in the bone metastasis group was significantly higher than that in the osteoarthritis group,and the difference was statistically significant(P<0.01).Conclusion The value of SPECT/CT SUVmax with bone metastasis was significantly higher than that of osteoarthritis,and the quantitative evaluation of SUVmax can supplement the qualitative analysis of bone metastasis and osteoarthritis.
