O型孕妇不同孕期IgG抗A/B抗体效价的动态监测

目的动态监测夫妇ABO血型不合的O型孕妇孕早期与孕晚期血清中IgG抗A/B抗体效价,探讨其在产后ABO-HDN诊断中的价值。方法采用微柱凝胶法对1158名夫妇ABO血型不合的O型孕妇孕早期与孕晚期进行IgG抗A/B抗体效价检测,分析其抗体效价的分布、变化幅度与产后ABO-HDN发生率的关系。结果 158例夫妇ABO血型不合的O型孕妇孕早期与孕晚期血清中IgG抗A/B抗体效价的分布与血型分布无相关性(P>0.05),产后ABO-HDN的发生率与产前(包括孕早期与孕晚期)IgG抗A/B效价的高低呈正相关性(P<0.05),与孕期IgG抗A/B抗体效价的升高幅度呈正相关性。结论动态监测夫妇ABO血型不合的O型孕妇不同孕期IgG抗A/B抗体效价,可更加准确的预测ABO-HDN的发生,为临床提供诊断依据。

Dynamic Monitoring of the O-pregnant Women during Pregnancy IgG anti-A/B Antibody Titer

Objective To monitor dynamically the serum IgG anti-A/B titer of O blood group pregnant women in different stages of gestation,to explore the diagnostic value of detection immune antibody in pregnant women with blood group incompatibility for preventing hemolytic disease of the newborn（HDN）.Methods 1158 pregnant women with ABO blood group incompatibility were determinates IgG anti-A or anti-B in first trimester and third trimester with micro-column gel agglutination assay,to analysis the relationship between postpartum HDN occurs and distribution of prenatal immune antibodies.Results It＇s no correlation of ABO blood type with pregnant women IgG anti-A or anti-B titer in first trimester and third trimester distribution in 1158 cases（P0.05）,IgG anti-A or anti-B titer and postpartum occurred ABO-HDN have correlation,with the antibody titer rising,the occurred rate is rising（P0.05）,the changing amplitude rising,the occurred rate is rising.Conclusion To monitor dynamically the serum IgG anti-A/B titer of O blood group pregnant women in different of gestation,and provide diagnosis evidence for predicting the occurrence of neonatal ABO hemolytic disease.
