2730例消化道内镜钳取组织粘膜印片的细胞学分析

目的评价细胞学方法诊断消化道良性及恶性疾病的优点和不足。方法通过消化道内镜钳取对2730例可疑组织进行细胞学诊断。结果2730例中,(1)发现恶性细胞434例,占15.90%,其中254例为胃癌;17例细胞学检查为恶性者,内镜检查为阴性,组织切片亦为恶性。(2)核异质54例,占1.98%;内镜检查未见异常,其中3例经病检为恶性。(3)良性病例(各种炎症、息肉、溃疡等)2242例,占82.12%;其中179例内镜检查为阳性者,经病理切片为恶性者150例,良性29例。结论细胞学检查方便、快捷,且可以补充内镜直视诊断和病理学的某些不足。但其准确性有赖于内镜取材和细胞学工作者的经验和诊断水平。

Cytological evaluation of 2730cases of alimentary canal mucosal tisues collect-ed by endoscope.

Objective To evaluate the cytological methods of diagnosing digestive tract diseases.Methods Suspected mucosal tissues collected from2730patients by endoscope were diagnosed by cytological imprinting.Results(1)Malignant cells were seen in434samples(15.90%),of which254were stomach cancer.Seventeen malignant cases diagnosed by cytology were negative by endosopy,but were diagnosed malignant histopathologically.(2)Heterokaryon was found in54cases(1.98%),but was negetive by endoscopy.Among them,3cases were testified malignant by pathology.(3)The rest2242cases(82.12%)were diagnosed benign in the179cases among them found positive by endoscope,150case were testified malignant and29benign histopathologically.Con-clu sions The cytololgical examination is relatively convenient and rapid,supplementing the deficiancy of direct examination by endoscope and pathology,But the accuracy of cytological diagnosis reries on the tissues collected by endoscope and the experience of the examiner.
