临终病人适宜性技术应用临床分析

目的研究和观察解除临终患者疼痛、呕吐、呼吸困难症状的适宜性缓和医疗技术方法和疗效。方法选自2007年1月至2009年6月本院安宁病房收治的120例晚期恶性肿瘤和老龄衰竭病例，对出现疼痛、呕吐和呼吸困难症状而用适宜性技术行缓和治疗，治疗方法保括：三阶梯镇痛；基本对症；中医药；脱水；特殊和心理治疗，单一治疗无效者作上述方法的综合治疗。结果经适宜技术治疗后的有效缓解率为：疼痛96．90％；呕吐60．61％；呼吸困难33．80％，无重大不良反应发生，治疗前后比较差异有统计学意义（P〈0．01）。结论本研究中使用的适宜性缓和医疗技术安全、有效、经济、操作便捷，缓解呼吸困难、呕吐方面疗效尚不够理想，在基层医疗单位尚有相当的适宜度。

The clinical analysis of the 120 patients with cancers approaching end applied with suitable technology

Objective To research and observe the suitable method and curative effects of the .relaxes medical treatment technology releasing the pain, vomiting and dyspnea of the patients approaching end. Method 120 cases were selected in the quiet ward of this courtyard, who had late period malignant tumor and old age exhaustion with the symptoms of pain, vomits and dyspnea from January 2007 to June 2009. They were treated with suitable technology relaxed cure. The method was three stairs of relieving pain： treating the symptoms of the illness basically, the deprivation of body fluids, special psychotherapy. If the single method had no effect, the synthesis mentioned above was used. Results The effective rate of the synthesis for relieving pain was 96. 90% vomits, 60. 61%; hypoventilation, 33. 80%. Seriously harmful reaction had not occurred. There was a statistical difference before and after treatment （P〈 0. 01 ）. Conclusion Relaxes medical treatment technology used in this research is safe, effective, economic, convenient. Although the treatment effect was inadequate ideally in the hypoventilation and vomiting, it is equivalent suitable in the basic level medical units.
