肺心病时血浆内皮素-1、心钠素和降钙素基因相关肽水平的变化

目的观察慢性肺源性心脏病患者血浆内皮素(ET)1、心钠素(ANP)和降钙素基因相关肽(CGRP)水平的变化。方法用放射免疫分析方法测定了肺心病急性发作期和缓解期患者血浆ET1、ANP、CGRP含量,并行同步动脉血气分析,且与慢性阻塞性肺病(COPD)组比较。结果肺心病患者血浆中ET1和ANP含量升高,CGRP含量降低,尤以其急性期为著。其中ET1和CGRP的变化不易随病情缓解而恢复,而ET1的变化与ANP呈显著正相关(r=0.425,P<0.05),与CGRP和PaO2呈显著负相关(r分别为-0.562、-0.741,P<0.01)。结论肺心病时血中ET1与ANP、CGRP水平失衡长时间存在,可能是该病病情加重的一个重要体液因素。

Changes of plasma endothelin-1,atrial natriuretic peptide and calcitonin gene-related peptide in patients with cor pulmonale

Objective Investigate the changes of plasme endothelin-1(ET-1) atrial natriuretic peptide(ANP)and calcitonin gene-related peptide(CGRP)in patients with cor pulmonale.Methods Immunoradiometric assay was used to measure plasma concentrations of the three sudstances in patients with cor pulmonale of acute stage and relieving stage.Simultaneouslg,artery blood gas analysis was done.Additionally,all of the parametres were compared with those of patients with chronic obstructive pulmonary disease(COPD).Results In patients with cor pulmonale,the levels of plasma ET-1,ANP incresed while the one of CGRP decreased,as was especially remarkable during the acute stage.Herein the levels of ET-1 and CGRP were not apt to restoring with the bettering of pathological situations.The level of ET-1 was significantly correlated positisety with the one of ANP(r=0.425,P<0.05),and negatively correlated with the ones of CGRP and PaO_2 (r=-0.562,-0.741,respectively,P<0.01).Conclusion The imbalance of plasma ET-1,ANP and CGRP longtermly existing in patients with cor pulmonale may be an important humoral factor deteriorating the pathalogical state of cor pulmonale.
