29例双胎之一宫内死亡的临床处理探讨

目的探讨双胎之一宫内死亡临床处理。方法对29例双胎之一宫内死亡的临床资料进行分析。结果23例期待治疗者中，阴道分娩15例，剖宫产8例，23例新生儿均成活，母体凝血功能未发现异常；6例双胎合并妊娠期肝内胆汁淤积症者均剖宫产，5例新生儿存活，1例死亡。结论无严重妊娠并发症的双胎之一宫内死亡后，可在严密监护下期待治疗至胎儿成熟，从而降低围产儿的死亡率；当合并有妊娠期肝内胆汁淤积症时一旦出现有一胎宫内死亡需立即终止妊娠。

Clinical Settlement of 29 Cases of Twins Pregnancy with Death of Either Twin

Objective To explore the clinical settlement of twins pregnancy with either twin. Methods Twenty-nine cases of twins pregnancy with either twin were reviewed retrospectively. Results Among 23 cases of expectant treatment, 15 samples were vaginal delivery and 8 samples were cesarean section. The 23 babies all were survived and the blood thrombin time mothers wer normal. Six samples of wins pregnancy at the same time had intrahepatic cholestasis of pregnancy were cesarean section, 5 sample neonatal succeed to give birth and 1 death. Conclusion Without pregnancy complications after the death of one of twins , we can expect the pregnancy may mature after close attention, so lower is the rate of death of new pregnancy; Once one of twins pregnancy is dead and at the same time there is intrahepatic cholestasis of pregnancy , we must stop pregnant
