髓芯减压联合应用胰岛素样生长因子I治疗激素性股骨头坏死

摘    要:目的 研究胰岛素样生长因子 (IGF I)在促进股骨头无菌性坏死自我修复中的生物学效应。方法 中国家兔 4 8只 ,分两次间隔 3周耳缘静脉注入马血清 (10ml/kg)。 2周后 ,泼尼松龙连续 3d腹腔内注射 ,建立股骨头无菌性坏死的动物模型。注射激素 2周后 ,行99mTc核素骨扫描 ,选择双侧股骨头血流量减少的动物 2 8只为下一步实验对象。动物随机分为 2组 ,每组 14只。对照组为单纯髓芯减压组 ,实验组为髓芯减压联合应用IGF I组。结果 实验组与对照组相比术后 1周 ,术后 2周骨钙素含量有显著性差异 ,术后 2周股骨头血流量有显著性差异。结论 IGF -I可有效促进成骨细胞转化 ,增加病变股骨头内血流量 ,促进了早期股骨头坏死的自我修复

Abstract:Objective To investigate the biological effects of insulin-like growth factor (IGF-I) on femoral head necrosis. Methods Forty-eight Chinese rabbits were received the intravenous injection of horse serum (10 ml/kg) twice at the interval of 3 weeks. After 2 weeks, prednisone (45 mg/kg) was injected for 3 days peritoneally. After 2 weeks of injection of prednisone, SPECT was used to select animals for next experiment which blood flow of femoral heads were decreased. Total 28 Rabbits were divided into 2 groups randomly. Control group was treated with core decompression, and experimental group core decompression and application of IGF-I. Results Compared with control group, the experimental group had a significant increase in serum osteocalcin after 1 or 2 weeks of operation, and blood flow of the femoral heads were increased after 2 weeks of operation. Conclusion IGF-I can enhance the activity of the osteoblast and prompt the restoration of early necrosisi of femoral head.
