短节段椎弓根钉固定手术治疗急性脊柱损伤的临床价值

目的探讨短节段椎弓根钉固定手术治疗急性脊柱损伤的临床价值。方法随机选取2012年5月~2015年5月济宁市第一人民医院收治的80例急性脊柱损伤患者,给予所有入选患者短节段椎弓根固定手术治疗,并对其手术前后伤椎椎体前后缘压缩高度及Cobb's角、脊髓神经功能恢复程度进行统计分析,并对其临床疗效进行评定。结果入选患者术后30min、1个月的椎体前缘压缩高度、椎体后缘压缩高度、椎管占位率、Cobb's角均显著低于术前(P0.05),但术后30min、1个月的椎体前缘压缩高度、椎体后缘压缩高度、椎管占位率、Cobb's角之间的差异均不显著(P0.05);术后1个月的Frankel分级中A、B、C、D级比例均显著低于术前(P0.05),E级比例显著高于术前(P0.05)。80例患者中,优37例,良35例,中6例,差2例,优良率为90.00%(72/80)。结论短节段椎弓根钉固定手术治疗急性脊柱损伤具有较高的临床价值,值得推广

Clinical value of short-segment pedicle screw fixation in the treatment of acute spinal trauma

Objective To study the clinical value of short-segment pedicle screw fixation in the treatment of acute spinal trauma.Methods 80 cases of patients with acute spinal trauma who were admitted to trauma surgery of our hospital from May 2012 to May 2015 were randomly selected.All patients were given short-segment pedicle fixation treatment.Then their compression height of front edge of vertebral body before and after surgery, Cobb's angle, spinal nerve functional recovery were statistically analyzed, and their clinical efficacy were evaluated.Results The vertebral compression height of the front edge, vertebral compression height of the posterior edge, spinal occupancy, Cobb's angle after 30min, 1 month of the enrolled patients were significantly lower than the preoperative (P<0.05), but the differences of vertebral compression height of the front edge, vertebral compression height of the posterior edge,spinal occupancy, Cobb's angle between after 30min and 1 month were not significant (P>0.05).The proportion of Frankel grade A, B, C, D levels after 1 month was significantly lower (P<0.05), while the ratio of grade E was significantly higher than preoperative (P<0.05).Among the 80 cases, 37 cases were excellent, 35 cases were good, 6 cases were fair, 2 cases were poor, the excellent and good rate was 90.00% (72/80).Conclusion The clinical value of short-segment pedicle screw fixation in the treatment of acute spinal trauma is high, so is worthy of promotion.
