微聚焦高能冲击波治疗骨质疏松性骨折的疗效评价

摘    要:目的探讨微聚焦高能冲击波治疗骨质疏松性骨折临床疗效。方法选取2013年1月~2014年1月笔者医院收治的60例骨质疏松性椎体骨折患者,根据其治疗方法将其分成对照组和观察组,每组30例患者。其中对照组患者仅给予非手术治疗,观察组给予微聚焦高能冲击波治疗。术后比较两组患者疼痛视觉模拟疼痛评分(VAS评分)、骨折椎体高度恢复率、Oswestry功能障碍指数、Cobb角、股骨近端骨密度以及不良反应发生率。结果观察组患者治疗后疼痛VAS评分(3.2±1.0)明显低于对照组患者(6.3±0.9),P0.05;观察组患者治疗后骨折椎体高度恢复率、Oswestry功能障碍指数以及Cobb角(23.9±12.1)%、(25.3±9.4)°和(14.2±4.1)°]均优于对照组患者(34.8±13.4)%、(56.8±11.2)°和(19.3±3.6)°],P0.05;观察组患者治疗后股骨近端骨密度(0.611±0.112)g/cm~2明显高于对照组患者(0.534±0.107)g/cm~2,P0.05);两组患者均未出现严重不良反应。结论微聚焦高能冲击波能够明显减轻骨质疏松性骨折患者的疼痛,改善临床指标,提高骨密度,预防骨量持续丢失,提高患者生活质量,值得临床推广应用。

Abstract:Objective To investigate the clinical efficacy of microfocusing high-energy shockwave in the treatment of osteoporotic fracture . Methods Sixty patients with osteoporotic vertebral bone fractures from Jan . 2013 to Jan.2014 in our hospital were divided into the control group and the observation group according to the treatment method.Patients in the control group were treated with conservative treatment ,and the observation group was treated with microfocusing high-energy shockwave .The postoperative pain VAS scores ,vertebral height restora-tion rate,Oswestry Disability Index,Cobb angle,proximal femur BMD and adverse reactions between two groups were compared.Results The VAS score after treatment in the observation group (3.2 ±1.0) was significantly lower than that of the control group (6.3 ±0.9),P<0.05)].The fractures of vertebral height restoration rate ,Oswestry Disability Index and Cobb angle after treatment in the observation group ( 23.9 ±12.1 )%, ( 25.3 ±9.4 ) °and (14.2 ±4.1)°] were better than those in the control group (34.8 ±13.4)%,(56.8 ±11.2) °and (19.3 ± 3.6)°,P<0.05].The BMD of proximal femur after treatment in the observation group (0.611 ±0.112 g/cm2)] was significantly higher than that in the control group (0.534 ±0.107) g/cm2,P<0.05].There was no serious adverse reaction in either groups . Conclusion Microfocusing high-energy shockwaves can significantly reduce pain,improve clinical parameters ,increase bone density and prevent bone loss ,meanwhile,it can improve the quality of life of the patients with osteoporotic fractures .It is worthy for clinical application .
