肺癌空洞X线、CT与MRI表现意义的分析

摘    要:目的:分析肺癌空洞X线平片、CT与MRI表现,探讨空洞表现与肿瘤性质、恶性程度意义。方法:对40例以各种方法证实为肺癌,40例均摄X线平片,其中30例做CT,8例做MRI检查。40例中30例手术,并做病理检查,病理结果与影像表现对照。结果:空洞内缘不规则似癌梁改变等、空洞壁厚度与空洞在肿瘤中位置,特别是肿瘤越小,且形成空洞或肿瘤越大,空洞偏大等均与肿瘤性质有关,CT对空洞内缘显示优于平片,且对肿瘤边缘显示有助于判定肿瘤恶性程度。MRI肿瘤组织T_1W和T_2W信号强度变化对肿瘤液化坏死、甚至对肿瘤性质有潜在定性能力。结论:肺癌空洞X线、CT与MRI表现和肿瘤性质有明显联系,空洞表现对诊断肺癌有帮助。

Abstract:Objective:To study the X-ray and CT and MRI appearances of the cavity in lung cancer and the relationship between the image features and the pathologic findings such as the properties and the malignant levels of tumor etc.Methods:40 cases with cavity formation in pulmonary carcinomas confirmed by cytology were included in this study.All patients underwent X-ray examination, 30 cases underwent CT and six cases underwent MRI examination also. The resected microscopic specimens were available for detailed observation in 30 patients whose lesion had been resected surgically.All image features of the tumor were observed in comparison with pathologic findings. Results:It was shown that the sign of irregular interior verge of cavity, thickness of cavity wall and the savity location in tumor, especially cavity in small tumor or larger cavity in big mass, were closely related to the properties of tumor.The interior verge of cavity was better displayed in CT than plain film, it would be useful in determining the malignant levesls of tumor to observe the margins of tumor.The signal characters of the tumor in T1-weight and T2-weight phasese of MRI have potential ability to demonstrate the necrosis of tumor and the properties of tumor even.Conclusion:The image features of cavity formation in lung cancer have close relationship with the properties of tumor, It would be valuable in diagnosis of lung cancer.
