社区居民血尿酸水平与代谢综合征各组分的相关性

[目的]探讨高尿酸血症(HUA)相关因素及其与代谢综合征(MS)的关系.[方法]选取2008年参加健康体格检查并资料完整的延吉市某社区居民886名,检测血尿酸(SUA)及相关生物化学指标,并同时测量体格指标,分析血尿酸与代谢指标间的关系及高尿酸血症的危险因素.[结果]男性与女性HUA患病率间差异有统计学意义(P<0.001),SUA水平与MS各危险因素均呈显著相关,其中与腰围、体重指数、收缩压、舒张压、三酰甘油、空腹血糖呈正相关,与高密度脂蛋白胆固醇(HDL-C)呈负相关;与正常SUA组比较,HUA组的上述各代谢指标明显升高(P<0.05),而HDL-C水平明显降低,同时除低HDL-C血症外,其他代谢紊乱的检出率均升高(P<0.05);随着代谢异常数目的增加,HUA患病率呈升高趋势(P<0.01);Logistic逐步回归分析结果表明,腰围、超重、高血压、高血糖、血脂紊乱是影响SUA水平的主要因素.[结论]SUA水平与多种代谢指标有相关性,提示HUA可能是MS的组分之一.在对多种代谢异常的预防与治疗中应注意SUA代谢异常所带来的不利影响.

Relationship between blood uric acid and metabolic syndrome in community residents

OBJECTIVE To analysis the related factors of high uric acid（HUA） and the relationship with metabolic syndrome（MS）.METHODS 886 of community residents of Yanji were selected in attended the physical examination and had intact data in 2008,and the HUA and its related biochemical index and body index were measured.RESULTS Prevalence of HUA was significantly different between female and male（P0.001）.The serum uric acid（SUA） level was significantly related to risk factors of MS,in which the waist circumference,BMI,SBP,DBP,TG and FPG was positive correlation and HDL-C was negative correlation.As compared to the normal SUA group,above index of HUA group were obviously increased（P0.05） while HDL-C level was decreased,and the detection rate of lipids disorder was higher in HUA group than the normal SUA group except low HDL-C（P0.05）.The prevalence of HUA had increased tendency with the increasing number of abnormal metabolism.Results of Logistic regression showed that waist circumference,overweight,hypertension,hyperglycemia,lipids disorder were the main influencing factors of SUA level.CONCLUSION SUA level is associated with metabolic index,indicating HUA may be one of the composing components of MS.It is worthy noticing the impact of SUA during the prevention and treatment of multiple metabolic disorder.
