孕妇血清HBV DNA含量与母婴宫内传播的相关性研究

摘    要:目的 探讨孕妇血清HBV DNA含量与母婴宫内传播的相关性.方法 将196例HBV感染孕妇依据其分娩前血清HBV DNA含量(>1×106和≤1×106copies/ml)分为两组,其新生儿出生后采用乙肝免疫球蛋白(HBIG)及乙肝疫苗(HBvac)治疗.检测两组孕妇分娩前血清HBV DNA含量、新生儿及7月龄婴儿血清HBV标志物(HBVM).结果 孕妇血清HBV DNA>1×106组和HBV DNA≤1×106组新生儿宫内感染率分别为42.7%和16.4%,两者比较差异有统计学意义(P<0.01).HBV DNA>1×106组婴儿7月龄时HBsAg 阳性率较HBV DNA≤1×106组高,而HBsAb阳性率较HBV DNA≤1×106组低,两组比较均有统计学意义(P<0.05).结论 孕妇血清中HBV DNA含量越高,发生母婴垂直传播的危险性越大,阻断效果也越差.

Abstract:Objective To investigate the relationship between serum hepatitis B virus DNA(HBV DNA) level of gravida and mother-to-infant transmission.Methods 196 HBV infective gravidas were divided into two groups according to their serum HBV DNA level before delivery(1×106 or ≤1×106copies/ml).Newborns of two groups received the treatment of hepatitis B immunoglobulin(HBIG) and HBV vaccine(HBvac).The serum HBV DNA level of gravida before delivery and serum HBV markers of all newborns and at 7th month of age were tested.Results The intrauterine infection rate of HBV DNA 1×106 group was higher than that of HBV DNA≤1×106 group(42.7% vs 16.4%,P0.01).With the HBV DNA≤1×106 group,the HBV DNA 1×106 group had more higher positive rate of HBsAg and lower positive rate of HBsAb(two P0.05).Conclusion The higher the serum HBV DNA level of gravida before delivery,the larger risk of mother-to-infant transmission and the poorer effect of interruption.
