73例迟发性血气胸的临床分析

本文报告闭合性胸部外伤后迟发性血气胸７３例，占同期闭合性胸部外伤住院病人的１６．８％（７３／４３３），其中同侧肋骨骨折６５例、对侧肋骨骨折３例、无肋骨骨折５例。作者对其诊断标准、形成机理及治疗等问题进行了讨论。部分病例经ＣＴ检查证实：壁层胸膜外或脏层胸膜下血肿破裂是造成迟发性血气胸的主要原因，强调迟发性血气胸治疗的有效措施是早期胸腔穿剌，胸腔闭式引流或剖胸探查术

Clinical Analysis of 73 Cases of Delayed Haemopneumothorax

Seventy three cases of delayed haemopneumothorax subsequent to closed chest trauma have been reported, occupying 16.8% (73/433) of all patients admitted due to closed chest trauma at the same time. Five patients had no fracture of the chest cage. Sixty five had contralateral rib fracture, the other three had ipsilateral rib fracture.The diagnostic standards,pathogenesis. and treatment were discussed . It was confirmed by CT in a part of cases that the main cause of delayed haemopneumothorax was rupture of hematoma of either parietal or visceral pleura. It is emphasized that effective treatments of delayed haemopneumothorax are aspiration or closed drainage or surgical exploration of pleural cavity
